    Kidney transplantation is the ideal treatment for end-stage renal disease patients. However, a severe shortage of transplant organs has led to an increase in ABO-incompatible living donor kidney transplantation, which is now an accepted method to increase the possibility of kidney transplantation. Recipients of the ABO-incompatible kidney transplantation, however, may experience complications due to desensitization by plasmapheresis and highly concentrated immunosuppressants before and after surgery. Some may have to go through a long and complicating period of hospitalization and treatments. Moreover, due to the fear of infection and transplant rejection after the transplantation, the patients experience difficulty in psychosocial adjustment.
    The purpose of this study was to examine what ABO-incompatible kidney transplant recipients experience before and after transplantation and to understand their experience in the personal, family, social and cultural context. A micro-ethnographic research was attempted to provide background data for the development of effective nursing intervention strategies which will actually help transplant patients adapt to their transplantations.
    Participants of this study were 10 ABO-incompatible living donor kidney transplant recipients over the age of 20 who have received the kidney transplantation surgery six months ago and who are currently outpatients at a general hospital in Seoul; six of the participants were male and among them, two were accompanied by their wives who were their donors, while four of the participants were female. The data was collected from December 23, 2015 until May 6, 2016 and was collected through a semi-structured questionnaire titled “What is your experience related with ABO-incompatible living donor kidney transplantation?” along with individual in-depth interviews, field notes from participant observation and available medical records. The methodology in Roper & Shapiro (2000) was used for this study and the data was analyzed by transforming and coding interviews, field notes and medical records into a description, classifying the data to verify patterns, generalizing the construction and theory, and by taking notes on personal reflections and thoughts.
    The findings of this study in regards to the unique experience and culture of ABO-incompatible living donor kidney transplant recipients were as follows.
    1. Donor relations between married couples were established after ABO-incompatible living donor kidney transplantation became possible, and recipients were in control with hopeful and positive thoughts after experiencing advanced medical technology.
    2. Participants experienced joy in hearing about the miraculous possibility of ABO-incompatible kidney transplantation and had high hopes for their new lives, but also suffered from the difficult process of finding their donors.
    3. Participants willingly endured the side effects and discomfort caused by desensitization treatments before their surgery, including the plasmapheresis and administration of immunosuppressants.
    4. Participants endured ABO-incompatible kidney transplantation despite of complications and acute rejections post-surgery, such as reoperation and treatment due to hemorrhage, renal failure and infections.
    The general experience of ABO-incompatible living donor kidney transplant recipients was as follows.
    1. The participants tend to be unaware of the severity of their disease and the discovery of their chronic kidney disease tends to be delayed due to spontaneous improvement in their symptoms.
    2. After being diagnosed with chronic kidney disease, patients initially reject modern treatments and desperately cling to complementary and alternative treatments in vain, only to experience frustration and hopelessness as the disease worsens.
    3. Due to physical, psychological, social pain and financial burden caused by dialysis, patients apply for deceased donor kidney transplantation and continue to live a worthless life, relying on the dialysis machine until receiving the transplantation surgery.
    4. Initially after their transplantation, patients strive to overcome physical discomfort, side effects of immunosuppressants, and to adjust to life after transplantation by managing their diet and everyday life. The social support from families, friends, and medical professionals are crucial at this point.
    5. After transplantation, patients tend to be happy about the changes in their bodies and minds, and live a changed, special life by spending leisure time with families, friends and loved ones, exchanging information and communicating through patients’ meetings, and by finding passion in their work. Patients are also concerned about the health of their donors and find the will and effort to maintain their transplanted kidney.
    In conclusion, the ABO-incompatible living donor kidney transplantation led to the formation of donation culture between couples. Participants were shown to achieve self-control with hopeful and positive ideas after their encounter with new medical technologies. The relationship between the ABO-incompatible organ transplantation recipient and donor became closer after the transplantation and both recipients and donors raised their concerns towards each other’s health. Participants who have lived a negative and passive life before the transplant were transformed into positive and active people after surgery.

    신장이식은 말기 신부전 환자의 가장 이상적인 치료법이지만 이식 장기의 심각한 부족으로 현대에는 혈액형 부적합 생체 신장이식이 확대되어 신장이식의 기회를 높일 수 있는 보편적인 치료법이 되었다. 그러나 혈액형 부적합 생체 신장이식 수혜자는 수술 전 후 혈장교환술과 고농도의 면역억제제를 투여하는 탈 감작치료로 합병증이 발생할 수 있고 입원기간이 길고 치료과정도 복잡하다. 또한 이식 후에는 감염과 이식받은 신장의 거부반응에 대한 두려움으로 사회 심리적 적응에 어려움을 경험한다. 이에 본 연구는 ABO 혈액형 부적합 생체 신장이식 수혜자들이 이식 전 후 일상생활 속에서 어떠한 경험을 하는지 그들의 경험 과정을 개인, 가족, 사회 및 문화적 맥락에서 심도 있게 이해하여 향후 이식 환자들이 이식생활에 적응하는데 실제적인 도움이 될 수 있는 효율적인 간호중재 전략을 수립하는 기초자료를 제공하기 위하여 일상생활 기술적 연구방법으로 시도되었다. 연구 참여자는 서울시 소재 일개 종합 병원에서 ABO 혈액형 부적합 생체 신장이식 수술을 받고 외래를 방문하는 수술 6개월이 지난 만 20세 이상인 남성 참여자 6명(공여자인 아내 2명도 함께 면담에 참여)과 여성 참여자 4명이었다. 자료 수집 기간은 2015년 12월 23일부터 2016년 5월 6일까지였으며, 자료수집 방법은 반 구조화된 질문지 “ABO 혈액형 부적합 신장이식과 관련된 경험은 무엇인가?”를 가지고 개별적 심층면담, 참여관찰을 통한 현장메모, 병록지 자료를 통해 이루어졌다. 자료 분석방법은 Roper & Shapira(2000)의 방법론을 사용하여 면담, 현장메모, 병록지를 서술로 구성된 서면화 된 글로 변형하여 코딩하기, 패턴을 규명하기 위하여 분류하기, 구성과 이론을 일반화하기, 개인적인 성찰과 통찰들을 기록하기 위하여 메모하기 등을 활용하여 분석하였다.
    연구결과로 ABO 혈액형 부적합 생체 신장이식 수혜자만의 독특한 문화와 경험은 다음과 같다.
    1. 부부기증 문화가 형성되었고, 의료 신기술의 체험으로 희망과 긍정적인 생각으로 자기조절을 하고 있었다.
    2. 혈액형 부적합 신장이식이 가능하다는 놀라운 기적의 소식을 접하고 새로운 삶에 대한 희망과 환희로 생체기증자를 찾는 과정의 수고로움을 겪으면서 이식에 대한 갈등이 교차 된다.
    3. 혈액형 부적합 신장이식을 받기 전에 고농도의 면역억제제 투여와 혈장교환 술과 같은 탈 감작치료의 부작용과 불편함을 기꺼이 감내한다.
    4. 이식수술 후 출혈로 인한 재수술 및 시술, 이식신장 부전, 감염과 같은 합병증과 급성 거부반응을 겪으면서 혈액형부적합 신장이식 수술의 어려움을 견뎌낸다.
    다음은 ABO 혈액형 부적합 생체 신장이식 수혜자의 전반적인 경험은 다음과 같다.
    1. 질병의 심각성을 알지 못하고 증상이 저절로 좋아져 만성 신장병의 발견이 늦어진다.
    2. 만성신장병 진단 후 현대의 치료법을 거부하고 완치를 위하여 절박한 심정으로 자연치유, 민간요법 등 보완대체요법에 매달리는 헛된 노력들도 하였으나 질병은 악화되어 좌절감과 절망감을 경험한다.
    3. 투석으로 인한 신체적, 정신적, 사회적 고통과 경제적 부담감으로 뇌사자이 식을 등록 하고 투석 기계에 의존하는 무가치한 삶을 살다가 이식수술을 받는다.
    4. 이식한 초기에는 신체적 불편감, 면역억제제의 부작용, 식이요법과 일상생활 관리를 위해 철저히 노력하면서 이식생활에 적응해 간다. 이때 가족, 친구, 의료인의 지지가 중요함이 나타났다.
    5. 이식 후에는 몸과 마음의 변화로 행복감을 느끼고, 가족과 친구 등 소중한 사람과 여유로운 시간을 보내고, 환우모임을 통한 정보교환 및 소통으로 서로 도우며, 일에 대한 열정이 생겨 몰두하게 되며, 기증자에 대한 고마움으로 건강을 걱정해주고 챙기며, 이식 받은 신장을 지키려는 의지와 노력이 강해지는, 변화된 새로운 삶을 살게 된다.
    결론적으로 ABO 혈액형 부적합 생체 신장이식은 부부기증문화를 형성하였고 참여자는 의료신기술 체험으로 희망과 긍정적인 생각으로 자기조절을 하고 있었다. 혈액형 부적합 수혜자와 기증자의 관계가 이식 후 더욱 친밀해 졌으며 서로의 건강을 걱정하고 챙겨주고 있었다. 참여자는 질병으로 이식 전에는 부정적이고 수동적인 삶을 살았으나 이식 후에는 긍정적이고 능동적인 삶으로 변화되었다.
