알츠하이머성 치매 (Alzheimer’s Disease, AD) 및 경도인지장애 (Mild Cognitive Impairment, MCI)는 조기 진단과 정확한 판별이 치료 및 예후에 결정적인 영향을 미치는 대표적인 신경퇴행성 질환이다. 이러한 질환의 진단과 진행 모니터링을 위해 뇌 자기공명영상 (Magnetic Resonance Imaging, MRI)이 널리 이용되고 있다. 초고해상도 7T MRI는 일반적인 3T MRI보다 정밀한 해부학적 정보 제공이 가능하나, 높은 설치 및 유지비용, 장비 크기와 자기장 차폐 요구사항 등의 물리적 제약, 그리고 높은 고주파 에너지로 인한 체내 흡수, 금속 삽입물에 대한 민감도를 포함하는 안전성 문제로 인해 실제 임상에서의 활용이 어렵다. 이에 따라 3T와 7T 영상 사이의 해상도 차이를 효과적으로 해소할 수 있는 대안적 접근법이 요구되고 있다.
본 연구에서는 초고해상도 7T뇌 MRI의 임상 적용 한계를 극복하기 위해 표준 3T MRI로부터 7T와 동일한 수준의 영상 (7T*)을 합성하여 AD 및 MCI의 진단 성능을 향상시키는 새로운 초해상화 (Superresolution, SR) 프레임워크를 제안한다. 본 프레임워크는 잠재 확산 모델 (Latent Diffusion Model, LDM)을 기반으로 하며, 자기지도학습 (Self-supervised Learning, SSL) 기법인 Barlow Twins를 활용하여 3T 및 7T 영상 간의 구조적 일관성과 표현 정합성을 효과적으로 학습하고 7T 수준의 해상도를 재현할 수 있도록 한다.
제안한 SSL 기반 Diffusion SR (SDS) 모델은 3T MRI로부터 추출된 Encoder Feature와 3T-7T 간 공통 구조 정보를 인코딩한 SSL Feature를 조건 입력으로 활용하여, 7T 상 미세 특징을 효과적으로 학습함으로써 해상도와 해부학적 일관성이 향상된 7T* MRI를 생성한다. 모델은 건강한 성인 50명의 3T-7T 데이터로 학습되었으며, SDS 모델의 성능은 정성적 시각화와 정량적 영상 품질 지표 및 해부학적 영역 기반의 Volumetric 분석을 통해 평가되었다. SDS로 생성된 7T* 영상은 원본 3T 영상에 비해 Peak Signal-to-noise Ratio 24.9%, Structural Similarity Index 9.8% 향상되었으며, Normalized Root Mean Squared Error 47.0% 감소되었다. 또한 모든 영역에서 기본 LDM 대비 좋은 결과를 나타냄을 확인하였으며, 외부 데이터셋에 대해서도 일관된 성능을 보여 모델의 견고성 및 임상 적용 가능성을 입증하였다. 특히 AD 조기 진단와 MCI예후 예측에서 중요하다고 알려진 해마의 CA1 영역이 SDS-7T* 영상에서 명확하게 구분되었으며, 이는 초해상화된 7T* 영상이 임상적으로 유의미한 세부 구조 정보를 보존하고 있음을 나타낸다.
생성된 7T* 영상을 이용해 AD 분류 성능을 확인한 결과, CNN 및 트랜스포머 기반 분류기 모두에서 SDS 기반 생성 7T* 영상으로 훈련한 모델이 정확도 및 민감도를 포함한 전반적인 성능 향상을 보였으며, 특히 트랜스포머 기반 분류기에서 가장 좋은 성능이 확인되었다. 이러한 결과는 SDS 모델이 해상도뿐만 아니라 질병 예측 정확도 향상에도 기여함을 의미하며, 7T MRI 장비 없이도 초고해상도 영상 기반 진단을 가능하게 하는 기술적 및 임상적 유효성을 보여준다.
다만 본 모델은 AD 환자가 아닌 건강한 성인의 3T–7T 쌍 영상만으로 학습되었기에 병리적 구조 변화에 대한 학습이 제한되며, AD 또는 MCI 환자의 쌍 영상 데이터를 확보하고 이를 통해 모델을 재학습한다면 진단 성능을 더욱 향상시킬 수 있을 것이다. 또한 분류에 사용된 데이터셋에서 AD, MCI, NC 간 클래스 불균형이 존재하였기에, 향후 연구에서는 데이터 수집 보완 또는 마이너 클래스 증강을 통한 편향 완화가 필요하다.
결론적으로, 본 연구는 7T MRI 하드웨어 없이 초고해상도 뇌 MRI 영상을 생성할 수 있는 새로운 접근을 제시함으로써, 생성형 인공지능 기반 의료영상 합성 기술의 임상적 활용 가능성을 입증한다. 또한, 초고해상도 MRI 기반 진단 정확도 향상과 영상 접근성 확대에 기여함으로써, 뇌 신경퇴행성 질환의 조기 진단 및 예후 예측의 정밀도를 향상시킬 수 있는 기반 기술로 활용될 수 있다.

Alzheimer’s disease (AD) and mild cognitive impairment (MCI) are prevalent neurodegenerative disorders, in which early and accurate diagnosis is crucial for effective intervention. Brain magnetic resonance imaging (MRI) is widely utilized for diagnosing and monitoring these disorders due to its ability to capture structural changes associated with neurodegeneration. However, the relatively limited spatial resolution of conventional 3T MRI often hinders the detection of subtle morphological changes, particularly in early disease stages. Although 7T MRI provides substantially improved resolution and detailed anatomical information critical for early diagnosis, its high cost of installation and maintenance, and physical constraints such as larger scanner size and magnetic shielding requirements limit its feasibility in general clinical environments. In addition, the need for specialized technical expertise and safety considerations including increased radiofrequency energy deposition and sensitivity to metallic implants further constrain its clinical adoption. As a result, despite its diagnostic advantages, 7T MRI remains inaccessible for the majority of healthcare settings worldwide, reinforcing the critical need for alternative solutions that can bridge the resolution difference between 3T and 7T imaging.
To address these challenges, this study proposes an advanced approach to generate 7T-equivalent (7T*) brain MRI from standard 3T scans by integrating self-supervised learning (SSL) with a latent diffusion model (LDM) that enhances AD and MCI diagnosis. Specifically, a self-supervised latent diffusion superresolution (SDS) framework was developed, combining the Barlow Twins method for extracting shared structural features between paired 3T-7T images, and a latent diffusion process that progressively reconstructs fine-grained anatomical details. By leveraging SSL for robust feature learning and the generative power of diffusion models, the SDS framework aims to emulate the high spatial fidelity of 7T imaging using only standard clinical 3T acquisitions.
The model was trained on a paired 3T-7T MRI dataset from 50 healthy adults and evaluated both qualitatively and quantitatively. Voxel-based morphometry analysis across 14 subcortical regions, including the hippocampus, thalamus, and amygdala, demonstrated high anatomical fidelity of the SDS-generated 7T* images. External validation on an independent dataset further confirmed the robustness and generalizability of the model across diverse cohorts. Image quality metrics showed substantial improvements with the SDS model, including a 24.9% increase in peak signal-to-noise ratio, 9.8% in structural similarity index, and a 47.0% reduction in normalized root mean squared error compared to 3T images. Additional enhancements over baseline LDM-generated outputs were also observed across all metrics.
Beyond image quality assessment, the clinical diagnostic utility of 7T* images was validated for AD classification. Classification models, including CNN and Transformer-based architectures, were trained and evaluated on both original 3T and SDS-generated 7T* images. The SDS-generated inputs led to notable improvements in classification accuracy for neurodegenerative disease, particularly with Transformer-based classifiers. These findings indicate that the SDS model enhances both qualitative and quantitative neuroimaging analyses, and facilitates earlier and more accurate diagnosis in clinical environments where 7T MRI is not available. Furthermore, this study demonstrates the potential of generative AI for medical image synthesis and superresolution, contributing to advances in medical imaging technologies.
