내시경적 Hemoclip 시술로 치유된 대장 게실 출혈 1예 = A Case of Colonic Diverticular Bleeding Treated with Endoscopic Hemoclip

Diverticular bleeding is a common cause of lower gastrointestinal bleeding in adults. Most of colonic diverticular bleeding stop spontaneously without any treatment. But diverticular bleeding develops at arterial vessel, which results in massive bleeding in a short period of time and may shows recurrence. Acute lower intestinal bleeding has been treated with heater probe coagulation, injection therapy, bipolar coagulation and endoscopic hemoclips. We experienced a 67-year-old woman presented hematochezia. Emergency colonoscopy was performed and showed a diverticulum with a visible vessel at the cecum. Hemostasis with hemoclip was successfully done. We herein report a case of hemostasis by capping a diverticulum of the colon with hemoclips. (Korean J Gastrointest Endosc 2002;25:228-231)

국문 초록 (Abstract)

대장 게실 출혈은 성인에서 하부위장관 출혈의 흔한 원인이고 대부분 자연적으로 지혈되지만 대량 출혈이나 재발이 가능하다. 하부위장관 출혈의 내시경적 치료에는 양극성 전기 응고술, 주사 경화요법, 온열 응고요법, 밴드 결찰술, hemoclip을 이용한 clipping 등이 있으며, clipping은 활동성 출혈이나 혈관이 보이는 경우 그리고 중증환자에서 궤양이나 천공의 합병증이 없이 비교적 안전하고 효과적인 지혈을 할 수 있다. 저자 등은 하부위장관 출혈로 내원한 67세 여자에서 대장내시경 검사를 실시하여 맹장의 게실강으로 노출된 혈관을 발견하고 clipping으로 치유된 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
