본 연구의 목표는 지역사회의 정상 및 비정상체중 여성의 신체건강상태를 조사 및 비교하고, 식행동 이상과의 관련을 분석하여, 식행동이상 여성에 대한 조기개입 및 비정상 체중으로 인한 신체이상 예방을 위한 기초자료를 제공하는데 있음. 연구내용으로 (1) 비정상 체중 여성 관련 국내 및 해외 동향을 파악하였으며 (2) 비정상 체중 여성 단면조사를 실시하였음. 구체적으로는, 가임기 (18세~39세) 지역사회 여성을 BMI에 따라, 저체중군 (체질량지수 10 percentile 미만, 18 kg/m²미만), 정상체중군 (체질량지수 10~90 percentile, 18~25kg/m²), 과체중군 (90 percentile 초과, 25kg/m²이상)을 분류하여 모집하였으며, 식행동이상장애 환자군의 임상적 특징과 비교하기 위해 신경성 폭식증군 및 신경성 식욕부진증군을 모집하였음. 검사항목은 생체시료 수집 및 검사, 이화학적 (biochemical) 검사 및 내분비계 이상(호르몬 등) 검사, 신체계측 및 건강행태 설문조사 (키, 몸무게, 체성분, 주관적 건강상태 등), 비정상 체중의 매개요인 파악을 위한 조사항목 (음주, 흡연, 신체활동 관련 설문조사) 등 임. 분석결과 저체중 여성에서 정상체중 여성에 비해 요추의 골밀도가 저하되어 있었으며, 여자대학생들의 골밀도는 체질량지수와 비례하였음. 한편 섭식장애 환자에서 골밀도는 신경성 식욕부진증 환자에서 현저히 낮았으며, 섭식장애 환자들의 골밀도는 체질량지수, estradiol, T3 와 비례하였음. 식이행동 질문지(DEBQ)의 절제된 섭식척도의 경우 정상, 과체중군이 저체중군에 비해 유의적으로 높았으며(p<0.001), 정서적, 외부적 섭식은 그룹간의 유의적 차이가 없음. 영양소 섭취상태를 분석한 결과 1일 총 섭취열량에서 세군간의 유의적 차이는 나타나지 않음. 정상체중군의 경우 대상자의 50%가 규칙적인 식사를 하고 있으나, 저체중과 과체중군의 경우 규칙적인 식사율이 낮은 것으로 나타남(p<0.05). 과체중군은 다른 체중군보다 과식 후 후회, 죄책감 등의 부정적 감정을 더 많이 갖고 있으며(p<0.001), 우울, 불안, 스트레스 시에 음식섭취량이 증가한다고 보고하였음(p<0.05). 본 연구결과 비정상체중 여성의 혈액 생화학적, 호르몬, 골밀도 이상, 정신·심리적 병리, 식행동에 관한 체계적인 현황 파악을 하였으며, 향후 각 세부 간 자료의 횡적분석을 통해 비정상체중 여성 및 식행동 이상 여성의 신체적 이상과 심리적, 영양적 특성간의 관련에 관한 규명이 가능할 것임. 이를 통해 향후 비정상체중 여성의 건강관리 및 교육지침 개발 등을 위한 근거자료로 활용할 수 있을 것으로 기대함.

The young women with low body weight is more common in Korea compared to US. The social pressure to be thin afflicts young women, who are getting involved in disordered eating behaviors to lose weight. Although overweight is more prevalent than underweight, underweight as well as overweight increases mortality and infertility in women. The aims of this study are to examine the medical status of young women with weight extremes, determine the relation with disordered eating behaviors by body weight. 3 young female groups of underweight (BMI < 10 percentile = 18 kg/m²), normal weight (10 percentile < BMI < 90 percentile, 18~25 kg/m²), overweight (BMI > 90 percentile = 25kg/m²) were recruited from universities students in Seoul. Patients with eating disorders were recruited from specialized clinic for eating disorders in Seoul. We examined demographic characteristics, psychological variables, and performed hematologic, biochmical, hormonal in all participants and bone density evaluation in the part of the participants by random sampling. In results, AST/ALT, glucose and T3 levels were increased in overweight women whereas WBC was decreased in underweight women. Bone mineral density in lumbar and femur were decreased in underweight women. The bone mineral density was correlated with the BMI in community samples. Overweight women had high scores in eating disorders psychopathology and were functionally impaired than normal or underweight women. Obese and under weight women were more likely to have lower frequency of meal regularity than normal weight group. Obese women had significantly more feeling of depression and guiltiness after overeating(p<0.001), and ate food more than usual when they feel negative emotion(p<0.05). Restrained eating score among DEBQ was significantly higher in obese women than normal and underweight group (p<0.001). Emotional eating and external eating scores were not significantly differences between groups. No significant differences in total energy intake were observed among the three groups. The present study suggest medical problems in underweight women and psychological impairments in overweight women in Korea. To improve public health, it is essential that appropriate and comprehensive approach to improve the eating attitudes and body image in young female.
