Background/Aims: Marry studies suggested that obesity, diabetes and cardiovascular risk factors are associated with colorectal cancer as well as adenomatous polyp. We analyzed to determine the association between increased arterial stiffness and colon adenomatous polyp after adjusting age, sex, waist circumference, body mass index, lipid profiles, fasting glucose and blood pressure. Methods: Database of 249 subjects (151 men and 98 women with mean age of 50.85±9.99 years) who underwent a check up of colonoscopy and pulse wave velocity (PWV) simultaneously at Dongguk University International Hospital. From July 2005 to September 2006 were analyzed. The subjects were classified into two groups, 200 cases of colorectal adenomas and 49 controls including normal, non-polyp benign lesions, or hyperplastic polyp. Advanced adenomatous polyps (n=10) were defined as subjects having over 3 numbers, over 1cm size, high grade dysplasia or villous component. Uni- and multi-variate analyses were done to identify risk factors for adenomatous polyp. Results: At uni-variate analysis, age, waist circumference, body mass index, heart-femoral PWV, mean brachial-ankle PWV were positively associated with adenoma. At entered multiple logistic regression model, not mean brachial-ankle PWV but heart-femoral PWV was significant risk factor 〔1.005 (95% confidence inetval:1.001-1.008), p=0.008〕 and waist circumference is borderline significant risk factor 〔1.064 (95% confidence interval:1.000-1.133). p=0.51〕 in adenoma group. In advanced adenoma group, only age was significant risk factor 〔1.096 (95% confidence interval:1.009-1.190, p=0.03)〕. Conclusions: Heart-femoral PWV reflecting on insulin resistance is increased in patients with colon adenomaous polyp. This results suggested that colon adenomatous polyp and increased arterial stiffness tend to be clustered in the same individual and these disease entities have a possibility to have the same etiology in common.
