국문 초록 (Abstract)

본 연구는 국내에서 작업치료사에 의해 시행되는 연하장애 재활치료의 현황을 파악하여, 작업치료사들의 의사결정을 도와 효율적인 근거 중심의 치료를 제공할 수 있는 연하장애 재활치료 프로토콜을 개발하기 위한 기초 자료를 제공하고자 한다. 본 연구는 2017년 6월 10일부터 6월 11일까지 대한연하재활학회의 ‘연하재활전문가 성인기본과정’을 수강한 작업 치료사들을 대상으로 배포·회수한 총 143부의 설문지를 분석에 사용하였다. 대상자의 임상경력은 3년 미만이 89명 (65.5%)으로 가장 많았고, 3~5년 미만 39명(28.7%), 5년 이상 8명(5.9%)이었다. 연하장애 재활치료는 치료 수요의 증대 로 인하여 전국의 재활병원 및 요양병원, 종합병원, 대학병원 등에서 다양한 치료적 테크닉을 바탕으로 활발히 시행되고 있 지만, 복잡한 임상의 치료 환경, 근거에 기반한 치료를 계획할 수 있는 시간의 부족, 치료사의 경력에 따라 제공되는 치료의 방법과 양에 큰 차이가 발생할 수 있다는 문제점을 안고 있었다. 임상의 치료 환경에서 다양한 증상의 환자에게 효율적으로 수행할 수 있는 근거기반의 연하장애 재활치료 프로토콜이 절실할 뿐만 아니라, 작업치료사의 경력 차이에도 불구하고 질적 인 연하장애 재활치료를 제공할 수 있는 프로토콜의 개발이 필요하다.

This study provides basic data for understanding the current state of dysphagia and rehabilitation treatments implemented by occupational therapists in Korea. The findings are expected to help clinicians develop a dysphagia treatment protocol and make clinical decisions. This study used a total of 143 questionnaires distributed and returned to work therapists who took the 'Adult Basic Courses' of the Korean Academy of Dysphagia Rehabilitation from June 10 to June 11, 2017. The dysphagia therapy is actively implemented using various therapeutic techniques in rehabilitation hospitals, nursing hospitals, general hospitals and university hospitals due to increased demand for treatment. However, occupational therapist had problems with complex clinical care environments, a lack of time to plan evidence-based treatments, and a considerable discrepancy in the method of treatments provided depending on the therapist's career. Since dysphagia therapy can be effectively used for a variety of patients in clinical care environments, a comprehensive and well-structured protocol is urgently needed. It is also necessary to develop a treatment protocol for dysphagia which can reduce the quality difference of treatment caused by the difference of occupational therapist's career.
