장애인의 경제활동참여에 따른 의료보장 적용유형 변화가 건강성과에 미치는 영향 : 성향점수매칭(PSM)과 이중차분법(DID)의 결합모형 적용 = Impact of Transition in Health Insurance Scheme According to Economic Activity Participation of Disabled People on Health Outcomes : The Application of Pooling Model of Propensity Score Matching(PSM) and Difference in Difference Method(DID)

국문 초록 (Abstract)

본 연구는 장애인 건강권 보장의 핵심인 의료보장제도의 체계유형별 제도효과를 건강성과 측면에서 평가하고자 하였다. 즉 기존에 의료급여 적용을 받던 장애인이 취업 등 경제활동참여를 통해 신규로 건강보험 적용을 받게 되는 의료보장 적용유형 전환이 있을 경우 장애인의 건강성과 지표로서의 미충족 의료와 건강관련 삶의 질은 어떻게 변화되는지를 실증적으로 평가함으로써 실효적인 장애인 의료보장을 구현하기 위한 전략 직조의 단초를 제공하고자 하였다. 이를 위해 대표성을 확보한 한국의료패널의 2015년~2019년 5개년도 자료를 기반으로 처치집단(의료보장 적용유형 변화집단)과 비교집단(의료보장 적용유형 지속집단)으로 구분하고 적용유형 전환 시점 전후의 건강성과를 고려하여 그 효과를 평가하고자 하였다. 이를 위한 분석은 선택편의와 내생성을 최소화하면서 전환에 따른 순수한 효과를 평가할 수 있도록 성향점수매칭(PSM)과 이중차분법(DID)의 결합인 매칭-이중차이분석법을 적용하였다. 분석결과, 취업 등 경제활동참여를 통해 의료보장 적용유형이 전환된 장애인의 경우 미충족 의료는 증가하고, 건강관련 삶의 질은 낮아지는 부정적 효과가 유의미하게 나타났다. 이에 의료보장 적용유형 전환과정상 의료급여와 건강보험 간 연계성 강화, 전환 결과상 건강보험의 보장성 강화, 의료보장정책과 취업촉진 등 고용정책과의 상호보완성을 고려한 정책강화를 위한 다양한 제언을 논의하였다.

다국어 초록 (Multilingual Abstract)

This study sought to evaluate the effectiveness of the health security system which is the core of guaranteeing the right to health for the disabled, by application type in terms of health outcomes. In other words, if there is a change in the type of health security system where the disabled who were previously covered by medical aid scheme are newly covered by national health insurance scheme through participation in economic activities such as employment, how will the unmet health care and health-related quality of life as health outcomes indicators of the disabled be affected? By empirically evaluating changes, I ought to provide a starting point for implementing effective health insurance for disabled. For this purpose, based on five years of raw data from 2015 to 2019 from the Korean Medical Panel, which secured representativeness, the group was divided into a treatment group(group whose application type of health security system was changed) and a comparison group(group whose application type of health security system was continued). I attempted to evaluate the effect by considering health outcomes before and after transition of the application type. The analysis for this purpose applied the matching-difference in difference analysis pooled propensity score matching(PSM) and difference in difference method(DID) to evaluate the pure effect of the transition while minimizing selection bias and endogeneity. As a result of the analysis, for disabled people whose application type of health security system was changed through participation in economic activities such as employment, there was a significant negative effect of increased unmet health care and decreased health-related quality of life. Accordingly, I discussed various suggestions to strengthen the linkage between medical aid scheme and health insurance scheme in the process of converting the application type of health security system, strengthening the coverage of health insurance in the result of the transition, and strengthening policies that take into account the complementarity of health security policies and employment policies such as employment promotion for the disabled.
