    유전자 변이의 임상적 분류는 임상 유전체 검사의 핵심 과정으로, 높은 정확성과 표준화된 해석 체계가 요구된다. 2015년 발표된ACMG/AMP 가이드라인은 전 세계적으로 널리 사용되어 왔으나, 고정된 근거 강도 체계와 일부 근거 적용 기준의 구체성 부족으로인해 일관된 임상 적용에 어려움이 있었다.
    ClinGen VCG (Variant Classification Guidance)는 변이의 임상적·기능적 맥락을 고려하여 근거 강도를 상향 또는 하향 조정할수 있는 유연한 체계를 제시하고, 근거 적용을 위한 구체적인 지침을 마련함으로써 변이 해석의 유연성과 명확성을 동시에 확보할 수 있게 하였다. 또한, 근거 강도 조정 과정에 Bayesian Framework 개념을 도입하여 변이 해석의 정량화를 위한 기반을 마련하였다.
    Bayesian Framework는 각 근거의 강도를 병원성 오즈(Odds of Pathogenicity, OP)로 환산하여 점수화하고, 변이 해석에 적용된근거들의 점수를 결합해 최종 분류를 도출하는 정량적 변이 해석접근법이다. 최근에는 이러한 접근을 통해 VUS (Variant of Uncertain Significance)의 비율을 감소시키고, 점수 구간에 따른 VUS의계층화를 통해 보다 체계적인 재분류 전략 수립에 기여할 수 있음이 보고되고 있다.
    본 종설에서는 ClinGen VCG와 Bayesian Framework를 중심으로 최신 변이 분류 전략을 정리하고, 효율적 임상 적용을 위한 생물정보학 도구 및 실무적 고려사항을 제시하고자 한다.

    The clinical classification of genetic variants is a critical component of genomic testing, requiring high accuracy and a standardized interpretation framework. The American College of Medical Genetics and Genomics (ACMG) and Association for Molecular Pathology (AMP) guidelines, published in 2015, have been widely adopted; however, their fixed evidence strength levels and the limited specificity of certain criteria have posed challenges in achieving consistent and reproducible clinical interpretation. The ClinGen Variant Classification Guidance (VCG) augments the ACMG/AMP framework by introducing a flexible system that permits adjustment of evidence strength according to the clinical and functional context of each variant.
    VCG also offers detailed recommendations for the application of individual criteria, facilitating greater clarity and consistency in variant interpretation.
    The Bayesian framework further enhances this process by translating each evidence strength into a quantitative metric—Odds of Pathogenicity (OP)—and combining the OP scores of all applied criteria to derive a final classification. This quantitative approach has shown significant potential for reducing the proportion of variants of uncertain significance (VUS) and enabling more systematic reclassification strategies through scorebased subcategorization of VUS. In this review, we summarize recent advances in variant classification with a focus on the ClinGen VCG and Bayesian framework, and we highlight key bioinformatic tools and practical considerations for their effective implementation in clinical practice.
