앤더슨의 의료이용 모형을 적용한 건강보험 스케일링 수진에 영향을 미치는 요인 = Factors Affecting the National Health Insurance Dental Scaling Experience Based on Andersen's Model

국문 초록 (Abstract)

본 연구는 앤더슨의 의료이용모형을 적용하여 2013년 7월 이후 전악 스케일링이 건강보험 급여로 전환된 이후 수도권 성인의 건강보험 스케일링에 영향을 미치는 요인을 파악하여 스케일링에 대한 접근성을 높이고, 수진율을 높이기 위한 정책의 기초자료를 제공하기 위하여 시행하였다. 을지대학교 기관생명윤리위원회로부터 윤리적인 승인(EU 14-28)을 얻었으며 2014년 7월 10일부터 9월 10일까지 20세 이상의 수도권 성인 250명을 편의 추출하여 자기기입식 설문을 하였다. 수집된 자료는 IBM SPSS Statistics 20.0(IBM Co., Armonk, NY, USA) 프로그램을 이용하여 분석하였고 제1종 오류수준은 0.05로 하였다. 연구대상자의 소인요인, 가능요인, 필요요인에 따른 특성은 빈도와 백분율로 산출하였고, 의료이용요인에 따른 건강보험 스케일링 수진 유무의 분포를 파악하고자 카이제곱 검정(chi-square test)을 실시하였다. 그리고 건강보험 스케일링 수진에 영향을 미치는 요인을 파악하기 위해 로지스틱 다중회귀분석(multivariable logistic regression analysis)을 실시하여 다음과 같은 결론을 얻었다.
1. 연구대상자의 건강보험 스케일링의 수진 유무의 분포는 성별, 혼인상태, 칫솔질 횟수, 구강위생보조용품사용유무, 흡연 유무, 음주 유무, 경제활동 유무, 단골치과 유무, 방문의료기관 유형, 스케일링 건강보험 급여화 인지, 치과 의료서비스 만족, 주관적 구강건강 인식, 치주병 진단 경험 유무, 최근 1년 이내 구강검진 횟수에서 유의한 차이가 나타났다.
2. 스케일링 건강보험 급여화 인지도 수준을 살펴 본 결과 여자, 연령 ‘40-65세 미만’, 하루 동안 칫솔질 ‘3회 이상’, 경제활동을 하는 경우, 치통경험과 치은출혈경험이 없는 경우 그리고 최근 1년 이내 구강검진 횟수가 2회인 경우에 높았다.
3. 건강보험 스케일링 수진에 영향을 미치는 요인은 소인요인에서 성별, 연령이 유의미하게 영향을 미치는 것으로 나타났고 가능요인에서는 치과 의료서비스 만족, 스케일링 건강보험 급여화 인지가 그리고 필요요인의 경우 주관적 구강건강인식이 유의미하게 영향을 미치는 것으로 나타났다.
따라서 건강보험 스케일링의 수진율을 높이기 위해 상기 요인을 활용한 홍보 프로그램과 정책의 개발이 필요하다.

다국어 초록 (Multilingual Abstract)

This study was conducted aiming at providing preliminary data for a policy to heighten accessibility to and use of dental scaling by understanding factors affecting National Health Insurance coverage dental scaling of adults residing in metropolitan area by applying Andersen’s behavioral model of health services use after dental scaling was converted to health insurance coverage from July 2013. This study obtained ethical approval from the Institutional Review Board(IRB) of Eulji University on July 7, 2014 and conducted a self-administered survey by extracting 250 adults over 20 years residing in metropolitan area through the method convenient sampling from July 10 through September 10, 2014. This study analyzed collected data using IBM SPSS Statistics 20.0(IBM Co., Armonk, NY, USA) program and set the level of type I error as 0.05. And this study conducted frequency analysis to analyze characteristics of subjects according to predisposing, enabling and need component, and also carried out chi-square test to understand distribution of National Health Insurance coverage dental scaling according to predisposing, enabling and need component, and obtained following conclusions by carrying out multivariable logistic regression analysis for the purpose of understanding factors affecting National Health Insurance coverage dental scaling.
1. With respect to the distribution of study subjects’ National Health Insurance coverage dental scaling, a significant difference came out according to the gender, marital status, times of tooth brushing, use of oral hygiene devices, smoking, drinking, socioeconomic activity, regular dental clinic, the Type of hospital, awareness of National Health Insurance coverage dental scaling, patient satisfaction, self-perceived oral health status, diagnosis of periodontal disease experience, the number of regular dental check-up(the recent one year).
2. As a result of investigating the level of awareness of National Health Insurance coverage dental scaling, this study found that the level was high among females, aged ‘40-64’, those who brush tooth ‘more than 3 times’ and take part in socioeconomic activities, without experience of toothache and gingival bleeding, and the ones who received oral examination more than 2 times within the most recent year.
3. As for the factors affecting National Health Insurance coverage dental scaling, study results also show that gender and age exert a significant influence in predisposing component, patient satisfaction, awareness of National Health Insurance coverage dental scaling in enabling component, and self-perceived oral health status in need component respectively.
Therefore, development of publicity programs and policies using the above component are considered need for heightening the rate of National Health Insurance coverage dental scaling.
