Intraarticular fracture of knee(tibial plateau or femoral condyle) occur primarily in a younger population and often the result of blunt trauma. Invasive treatment optioens have historically included a wide variety of surgical methodologies, but these treatments have been associated with significant complications such as joint stiffness, malunion, infection, loss of fixation, skin loss and amputation. these may be associated with lengthy operative procedure and extensile surgical approach through tenuous soft tissue envelope. So we applicate small wire circular external fixator in percutaneous fashion with minimal additional devitalization of bone, periosteal blood supply and endosteal blood supply.
The authors analysed the 8 cases of intraarticular fractures of the knee joint that reduced by Ilizarov external fixator from April 1992 to August 1995. The fixator can be designed to compress the fracture gap to achieve bone to bone contact without additional bone grafting. Fracture gap secondary to malalignment can be corrected sequentially as bone union take place.
The following result were obtained.
1. All fractures have healed and mean union time was 15.5 weeks.
2. If there is severe comminuted open fracture or soft tissue injury, open reduction and internal fixation method is inadequate.
3. In theses case, arthroscopic joint Line realignment and Ilizarov external fixator application can get good result.
