장기요양보호시설에서의 사례관리실천 적용에 관한 연구 = 노인요양시설 거주 노인에의 단일사례분석

Korea has experienced rapid change to the aging society. Thus, long-term care for the elderly becomes important more and more in Korea. In this paper, through an evaluations of the case management practice application, I intend to look for implications for the effective and efficient case management model for the elderly in long-term care and nursing home. Applied case management model in this study has a special feature as a counselling centered approach and socio-psychology centered approach, instead of health care centered approach. And also, applied case management model focus on intervention to strengthen clients` informal relationship with family member, relatives, and friends in community. This case management model was applied to an elderly client in long-term care and nursing home from November, 1999 to December, 2000. According to the final evaluation, application of this case management model is turn out to be successful. The client rapidly adapted herself to the new environments successfully. And, owing to the continuous individual counselling and support, she could overcome many problems which occur to the nursing home clients in the initial stage. She could deal with dist ness well. And, she could build up and maintain positive relationships with peers. Moreover, she could enhance the social function ability. Especially, owing to the continuous intervention to intensify client`s informal relationships with her friend, she could have a strong will to resident in community. Through the case management process, she recovered an ability to live for herself. Though above results have an limit to generalize due to the case study, but these results suggest such implications to the social work practice for the elderly in long-term care and nursing home. First, case management is turn out to be an effective practice in this field, so it is necessary to introduce an case management practice actively, especially case management with an counselling centered and an socio-psychology centered approach. Second, interventions to strengthen client`s social functions are helpful to recover the ability to live for herself. So as to enhance rehabilitation of the long-term cared elderly to community, it`s necessary- to intensify these intervention through case management process. Third, this study find out that intermediation programs which focus to an maintenance of relationship with privately related persons is important to rehabilitation. So, in order to apply the case management successfully, it`s necessary- to develop these programs more and more and manage them more actively.
