In this country the case in which the procedure for aortic annular enlargement is necessary was rare. One patient at our department had severe mitral stenosis and moderate aortic regurgitation with small aortic annulus. Aortic annulus was less than 17 ㎜ in diameter after excision of valve leaflet for aortic valve replacement. Aortic annulus was enlarged with Manouguian technique and aortic and mitral valve replacements (St. Jude Med 19 ㎜ and 27 ㎜, respectively) were performed. This enlargement procedure appears to be satisfactory for double aortic and mitral valve replacements in adult patient with a small aortic annulus.
