출산의 의료화와 여성의 재생산권

국문 초록 (Abstract)

출산의 의료화는 여성과 아이의 건강과 안전에 유용하기도 하였지만 의료 권력에 의해 여성의 재생산 권리가 침해되는 결과를 초래하였다. 본 연구에서는 신체 자결권과 신체 통합권이라는 재생산 권리 개념을 도입하여 어떠한 여성들의 권리가 규제되고 있는지, 여성들은 이에 어떻게 저항하고 있는지를 다루고자 하였다.
여성들은 출산 서비스를 이용하게 되면서 자신의 몸과 아이에 대한 충분한 정보를 제공받지 못하고 있다. 여성들에게는 의학적 조치를 취할 때도 의료진이 제시한 결정을 수용하는 제한적인 결정권이 있을 뿐이다. 산전관리와 분만의 과정에서 여성의 몸은 기계와 같이 다루어지며 인격체로서 존중되지 않고 있다. 여성들에 대한 정서적 지원이나 배려가 부족하며 여성의 출산 능력을 인정하지 않는 경향이 있다. 이는 여성들의 출산 경험에서 상실감과 무력감을 느끼게 하는 요인이 된다. 이에 여성들은 여성의 권리에 대해 새롭게 인식하고, 여성의 경험을 중심으로 출산을 정의하기 시작한다. 여성들 간의 집단적 연계망을 통해 이러한 인식은 보다 확고해 지며, 이를 통해 기존의 의료체계 내에서 자율성을 확대할 수 있는 전략적 저항과 의료체계를 넘어서는 전면적 저항-조산사 분만-으로 이어진다.
본 논문은 의료된 출산 관행의 문제점을 여성의 권리 차원에서 부각시킬 수 있으며, 여성 중심적 출산 관행이 되기 위해서는 출산 조력자와의 비위계적 관계 형성, 정보의 수평적 공유, 다양한 수준의 출산 조력자-의사, 조산사, 둘라-등의 참여를 제안하게 된다.

다국어 초록 (Multilingual Abstract)

This article is to examine how women's reproductive right is regulated by authoritative medical professions and in what condition women can challenge the dominant medicalized childbirth practice.
Women have experienced violations of their bodily self-determination right such as information supply focused on medical experts, hierarchical distribution of information, and lack of decision-making right in medical monitoring and intervention. And, women's body has been handled machine-likely with respect to use of medical services and the bodily integrity right has been also restricted by the lack of respect for women's reproductive capacities and insufficient support for their emotional needs. These violations of women's reproductive rights cause women under pregnancy and childbirth to be disempowering.
Some women have started to challenge the medicalized childbirth. Women try to redefine their reproductive process as an natural physiological process and begin to ensure their reproductive capacities. Their resistance varies from going to hospital lately, not speaking of physical pain to the medical staff, moving and relaxing freely to changing the medical attendant into the midwifery.
It will be suggested that women should be recognized as an autonomous subject in their childbirth and horizontal relation between medical experts and pregnant women shall be established. More effort to provide various delivery attendants and services- such as midwifery, doula-will also be recommended.
