자경경부상피내 종양 및 유사병변의 MIB-1 발현 = MIB-1 Expression in Cervical Intraepithelial Neoplasia (CIN) and CIN-Mimicking Lesions

국문 초록 (Abstract)

자궁경부 편평상피암종은 우리나라에서 흔한 악성 종양으로서 전암성 병변인 자궁경부상피내 종양(CIN) 등급의 결정은 환자의 치료 방향 및 예후 결정에 매우 중요한 인자이다. 종양의 증식능 판정시 유용한 표지자인 MIB-1에 대한 면역 조직화학염색을 자궁경부의 종양성 및 비종양성 편평상피성 병변에 적용시켜 CIN의 등급 판정과 CIN 유사병변의 감별에 있어서 유용성을 살펴보았다.

다국어 초록 (Multilingual Abstract)

The aim of this study is to evaluate the significance of MIB-1 immunostaining in the grading of CINs and in the distinction between CIN and CIN-mimicking lesions (koilocytotic atypia and atypical immature squamous metaplasia). The materials were fifty eight cases, composed of 39 CINs and 19 CIN-mimicking lesions. At first, three pathologists had examined H &E section independently and interpreted the immunohistochemical findings for MIB-1. The interobserver discrepancy on H&E sections was noted in 7 cases. Later additional three pathologists reviewed these seven cases, and then all pathologists reexamined together for the diagnostic consensus. In CINs, the distribution pattern of MIB-1 positive cells was well correlated with the histologic grades. There was an overlapping MIB-1 expression between CIN I and CIN-mimicking lesions. MIB-1 findings of florid Koilocytotic atypia or Atypical immature squamous metaplasia were sometimes confusing due to similarity to high grade CINs. In conclusion, MIB-1 immunostaining could be used as a helpful method for the grading of CINs, and is not reliable for distinction between CINs and CIN-mimicking lesions.
