종합병원에 입원한 노인환자의 낙상위험요인 = Falls Risk Factors of Elderly Inpatients

The purpose of this study was to identify the fall risk factors and to suggest data for developing a program for preventing falls among elderly inpatients. The medical records of a total of 720 elderly patients over the age of 60 yr who admitted to hospital from May, 2010 to September, 2013 were reviewed. Three hundred forty patients who had experienced fall were matched with 340 patients who have never experienced fall. The fall information was obtained from electronic medical records and fall reports. There were significant differences in age, fall history, elimination, medication, mobility, cognition at admission between fallers and nonfallers. Logistic regression analysis was performed to elucidate the factors that influence falls. The probability of falls was increased by fall history, medication, mobility disturbance. When falls did occur, these occurred within one week following admission, in the patient room, among patients with alert mental status, and among patients who were ambulant with some assistance. This finding can be used as a useful resource to develop nursing intervention strategies for fall prevention at the hospital.

국문 초록 (Abstract)

본 연구는 종합병원에 입원한 노인환자의 낙상위험요인을 파악하여 임상에 효과적이고 적합한 낙상예방프로그램 개발의 기초자료를 제공하고자 시도된 서술적 조사연구이다. 연구대상자는 B시에 소재한 1개 종합병원에서 2010년 3월부터 2013년 9월까지 입원한 60세 이상의 노인환자 중에서 처음 낙상이 발생하여 간호 단위에서 병원의 질(quality) 관리 부서로 낙상보고가 이루어져 낙상발생 보고서를 제출한 360명을 낙상환자로 선정하였으며, 비낙상환자는 2010년 3월부터 2013년 9월까지 퇴원한 60세 이상의 노인환자 중 등록번호를 보고 난수표를 이용하여 360명을 선정하였다. 연구도구는 존스홉킨스병원 낙상위험 사정도구(Johns Hopkins fall risk assessment tool, JHH), 낙상발생 보고서이며, 전자의무 기록지를 통해 자료를 수집하였다. 자료분석 방법은 SPSS 18.0 프로그램으로 빈도, 백분율, 평균과 표준편차, 카이제곱검정, t-검정, 로지스틱 회귀분석으로 분석하였다. 낙상에 영향을 미치는 요인으로는 낙상경험(OR:25.181, CI:9.386-67.559), 약물복용(OR: 2.091, CI: 1.035-4.225), 보행장애(OR: 3.161, CI: 1.618-6.178)로 나타났다.
