A Case of Spinal Cord Cavernoma Mimicking Transverse Myelitis

국문 초록 (Abstract)

척수의 해면 혈관종은 척수에서 생기는 드문 병변이며 소아에서 특히 드물다. 저자들은 하지의 근력저하와 감각소실, 배뇨장애를 호소한 14세 여아로 급성 횡단성 척수염으로 치료 받은 후 호전되었으나, 11개월 후 증상 재발하여 시행한 자기공명영상 소견상 척수의 해면 혈관종으로 진단받고 수술적제거를 받은 후 신경학적 증상의 호전을 보인 1례를보고하는 바이다. 소아에서 급성 사지 마비 증세를보일 경우, 드물지만 감별진단으로 척수의 해면 혈관종 또한 고려해 보아야 할 것으로 생각된다.

다국어 초록 (Multilingual Abstract)

A 14-year-old girl presented with recurrent, progressive lower leg weakness and decrease in sensation. When the symptoms first appeared, she was misdiagnosed as transverse myelitis and was treated with methylprednisolone. Eleven months later, when she had the same symptoms, spine magnetic resonance images showed the characteristic findings of spinal cord cavernoma and she underwent surgical intervention. Spinal cord cavernomas are rare lesions. Because of their highly bleeding tendency, acute neurological deterioration occurs secondary to hemorrhage within the spinal cord. In order to prevent intramedullary hemorrhage and to halt the progressive
neurological decline, surgical resection is recommended. We experienced a case of spinal cord cavernoma presenting with recurrent transverse myelitis clinically.
