大田地域 社會福祉士의 事例管理的 接近에 대한 認識과 活用에 관한 硏究 = (A) Study on the Recognition and Application Regarding the Social Worker's Case Management Approach in Daejeon, Korea

국문 초록 (Abstract)

사례관리는 서비스의 누락과 중복을 예방하기 위하여 클라이언트의 욕구에 보다 초점을 두어 면밀한 사정과 계획을 한 후, 기관의 범위를 넘어 지역사회의 공식적 또는 비공식적인 자원을 보다 적극적으로 제공하고 점검을 강조하는 것이다. 즉, 종합적이고 비용 효과적인 서비스 제공이 가능한 실천방법이다. 그러나 아직까지 우리 나라에서는 사례관리의 개념, 목적, 기능과 역할 등에 관한 연구와 한국적 적용가능성을 검토하는데 그치고 있다.
이에 본 연구는 최근 사회복지분야에서 관심이 높아지고 있는 사례관리의 개념을 정리하고, 지역복지실천현장에서 사례관리의 인식정도와 활용 현황을 파악하고자 하였다. 사례관리를 실천하는데 있어 필요한 기반조성에 대해 고찰하여 좀 더 효율적으로 활성화시키는 방안을 연구하고자 한다.
위의 연구목적을 달성하기 위해 본 연구에서는 사례관리와 관련된 선행연구를 바탕으로 연구모형과 연구문제를 구성하였다. 그리고 지역사회복지관의 사례관리 실천에 대한 이해와 정도를 측정하기 위한 설문지를 구성하였다.
본 연구의 조사대상은 대전 소재 지역사회복지관의 사회복지사이다. 분석에 사용된 자료는 88부의 응답자료이며 SPSS 통계 패키지를 이용하여 분석한 주요 연구 결과는 다음과 같다.
첫째, 사회복지사들의 사례관리적 접근에 대한 전반적인 인식정도이다. 사례관리적 접근의 개념 인식은 사례관리적 접근의 실천과정과 서비스내용을 실천하는데 있어 보통이다 이상에 응답하여 사례관리적 접근이 무엇인지 제대로 인식하고 있음을 확인할 수 있었고 인식 또한 높은 것으로 나타났다. 구체적으로, 사례관리적 접근의 필요성 인식은 응답자 91% 이상이 필요하다고 응답하였다. 사례관리적 접근의 클라이언트의 문제해결에 대한 도움정도에는 응답자 모두가 도움이 되는 편이라고 응답하였으며, 다른 실천방법과의 비교했을 때의 효과성 인식 또한 높다고 인식하고 있었다(62.5%). 사례관리적 접근이 유용한 클라이언트의 문제는 선행연구와 마찬가지로 복합적이고(62.5%) 장기개입을 요하는 문제(55.7%)가 절반이상을 차지하였다.
둘째, 사회복지사의 사례관리적 접근에 대한 활용정도는 다음과 같다. 사례관리적 접근을 활용하고 있거나 활용한 경험이 있다고 응답한 사회복지사는 전체 88명 중 50명으로 나타나, 사례관리적 접근의 활용정도가 응답자의 절반이상을 차지하고 있는 것으로 나타나 아직은 활발히 활용되고 있지 않음을 보여주었다. 그러나 활용하고 있거나 활용한 경험이 있는 사회복지사 50명의 경우, 사정-계획-서비스조정-모니터링 및 재사정을 모두 실시했을 경우를 사례관리적 접근을 했다고 본 연구에서 보았을 때, 응답자 전원이 활용하고 있는 정도가 높은 편으로 응답하였다. 그 외에 세부적인 서비스 내용 또한 환자·가족 교육훈련을 제외하고는 역시 높은 편으로 나타났으며 특히 직접적 서비스의 제공은 가장 높았다. 그리고 일반적인 실천과정에 비해 절차나 형식도 제대로 갖추고 있었으며, 욕구조사 등이 더 활발하며, 활용정도 또한 높은 것으로 나타났다. 그 밖에, 사례관리적 접근의 사정방법은 주로 가정방문과 전화 등을 이용하고 있었으며, 피드백 횟수는 주로 필요시마다 하는 것으로 나타났다. 사례관리적 접근의 계획 및 사정의 주체로 계획은 담당사회복지사, 사정은 팀이 주체가 되어야 한다고 응답하였다. 또한 평가주체는 사례관리적 접근에 참여한 모든 이가 되어야 한다는 응답이 절반을 넘었다. 분야별로 활용되고 있는 정도는 재가복지와 노인복지, 장애인복지 순으로 나타났으며, 분야별 효과성 인식 또한 노인복지, 재가복지, 장애인복지의 순으로 나타나 선행연구의 결과와 별다른 차이가 없는 것으로 나타났다.
셋째, 사례관리적 접근을 위해 필요한 기반조성의 경우, 전문가 확보가 가장 높았으며 그 다음으로 자원들간의 연계, 지역사회자원, 비공식자원의 순으로 나타났다. 또한 사례관리적 접근을 하고 있지 않은 이유에서 시간과 재정부족(76.3%)이 월등히 높은 것으로 나타나 재정확보가 최우선시 되어야할 것으로 보인다.
이상의 연구결과로 볼 때, 사례관리적 접근에 대한 효과성 인식과 활용도는 매우 높으며, 분야별로 나누어 보면 선행연구와 동일하게 노인복지, 재가복지, 장애인복지이다. 이러한 분야에서 사례관리적 접근이 유용하게 활용되기 위해서는 사례관리적 접근을 위한 자원을 찾아내고 조정하는 개발기능과 재정확보가 시급히 요청된다고 할 수 있다. 즉 사례관리적 실천에 걸림돌이 되는 시간과 재정부족, 인력부족이 원만히 해결되어 전문가가 확보되어 사례관리적 접근의 기반이 조성되고, 사례관리자에 대한 보수교육이 원활히 이루어질 때 사례관리적 접근에 대한 효과성과 활용도는 더욱 높아질 것이다.
이상의 연구를 토대로 앞으로의 사례관리에 대한 연구에서는 종단적 연구나 질적 연구를 시도하는 것이 필요하고, 사례별로 집중적으로 연구하여 사례관리적 접근의 효과성 검증이 이루어져 좀 더 효과적으로 클라이언트의 문제 해결에 활용되어야 할 것이다.

다국어 초록 (Multilingual Abstract)

The study focuses on the needs of clients to prevent an accumulation of services. It is based on specific cases and their plans, and their range is beyond an institution. The study emphasizes the check of the formal of in formal resources in this region. So does the supplement of more active and better services. The research needs the supplement of services that are synthetic and cost-effective. But in Korea, it's all to study a concept, a purpose, function, and the roles of case management, and their applyment's ability.
Therefore at first I will define case management and investigate its degree of recognition and use in real situation in this study. Also, to practice case management, I'll go into their fundamental tasks. Lastly I'll study that the way welfare center's work effectively.
In this study, to achieve these purposes, I composed research of pre-study materials related with case management. I also made a survey to investigate the understanding of case management's practice.
This study's target is social workers who are in community welfare centers. In the survey, a total of eighty eight persons responded, and I analyzed their answers using a statistics package. The result follows:
First, questions dealt with what the social workers thought about the practice of the case management approach. Most people had a positive concept of the case management approach. In the item asking if the case management. Approach was needed responder answered that it was absolutely needed(91%). Therefore the degree of need was considered high. Clients' problem in which the case management approach was used usefully, required the clients' synthetic with and long period's of intervention, like pre-study. Over half of the responders agree that. The case management approach is very helpful in solving clients problems, compared to other practice methods. Generally, respondents were satisfied with the effectiveness of case management(62.5%). It's turned out that half of the Welfare workers think that the case management approach is effective.
Second, in the degree of the practice of social welfare, regard for the home care service was the highest concern and, next highest was welfare for the old, welfare for the handicapped. Also in the perceived degree of the effectiveness in every field, the welfare for the old was the highest, and the welfare for the handicapped was the next. This result is no different from the pre-study. According to the case management approach, all respondents are assessing, adjusting, plann-ing, monitoring and reassessing. Besides it proved that the degree of application was high. The other services also is on the high side except for the patient, family training. Especially, providing direct service was the highest. It is well equipped with procedure and format compared with another processing, and the survey was active and the degree of research application was high. The case management
approach uses almost exclusively home-visiting and phone calling. Respondents answered that social workers should make a plan for the case management approach, and also a team should be the chief object of a case management approach. Half of the respondents answered that all the members who are taking part in the case management approach should be the chief object of evaluation.
Third, in order to putting case management approach on a solid foundation, these are the requirements; an expert having professional education, being closely connected with resources, community social resources, and informal resources. Social worker do not use the case management approach because of the shortage of time and lack of money(73.6%). Therefore, making sure of the money is urgent.
In short, it is high that effectiveness and application about case management approach. In the elderly welfare and the handicapped person welfare, finance secure and development is requested for achieving the application of case management approach. Namely, in this field the weak finance, time shortage, social worker's deficiency are problems. If these problems are solved effectively. The effectiveness and application of case management approach will be much higher than before.
In the future, the study of case management should be in quality. The case management approach must solve the client's problems effectively.
