의료급여비 지출의 변동패턴과 결정요인 = The Pattern and Determinants of Government Expenditures on Medical Aid Program

국문 초록 (Abstract)

본 연구는 30년의 역사를 가지고 있는 한국의 의료급여비 지출변동이 어떠한 특성을 지니고 있으며, 지출변동에 영향을 미친 결정요인은 무엇인지를 양적 분석을 통해 밝히고 있다. 먼저 1977~2007년 동안 한국의 의료급여비 지출은 연평균 25.3% 증가하였으며, 지출변동률의 분포는 정규분포보다는 첨도(5.9)가 크고, 꼬리가 긴 leptokurtic 분포에 가까운 것으로 나타났으며, ±30%를 점증성 기준으로 할 때 73.3%가 점증적 변동이었던 것으로 나타났다. 다음으로 지난 30년의 의료급여제도의 역사와 지출변동을 검토한 결과, 의료급여비 지출규모는 정부의 정책의지에 따라 변동하였던 것으로 밝혀졌다. 마지막으로 회귀분석의 결과, 의료급여비 지출변동은 사회경제적 요인, 정치적 요인, 정부체제적 요인의 변동에 의해 유의미하게 설명될 수 있었다. 지출변동률을 종속변수로 하고, 인구증가율, 노인인구증가율, 실업률, 물가상승률, 선거실시여부, 국회여소야대여부, 정책변동수준 등을 독립변수로 하는 중회귀분석모형은 통계적으로 유의미한 것으로 나타났으며, 특히 정책변동수준과 노인인구증가률이 통계적으로 유의미한 영향을 미쳤던 것으로 나타났다. 향후 의료급여비 지출수요는 고령화의 진전, 소득양극화의 심화 등으로 지속적으로 증가할 것으로 보이며, 정부의 정책결정은 실질적인 의료급여비 수요를 충족시키되, 예산낭비의 원인으로 지적되고 있는 의료급여 수급권자와 의료서비스 공급자의 도덕적 해이를 줄이기 위한 방향에 초점을 맞출 필요가 있다.

This study tried to find out the characteristics and determinants of change of government expenditures on Medical Aid Program (MAP) in Korea. Distribution of the rate of change in medical aid expenditures was near to leptokurtic, not to normal distribution, i in 1977-2007. The rate of change could be explained by socioeconomic, political, and governmental system factors. Change in government policy and the increasing proportion of aged people were important factors in the regression model. Historical analysis of the past 30 years of MAP showed that government was able to control the level of expenditures with policies related to the number of entitled people, the price of medical service, and the behavior of entitled people and medical service providers. The demand for MAP expenditure will increase according to the increase in the number of aged people, and deepening socioeconomic polarization. The Korean government should not only prepare for the increase in demand, but also policies to curb moral hazard behaviors of entitled people and medical service providers.
