Purpose: This study aimed to identify internal and external workplace violence experiences among Emergency Medical Technicians (EMTs) and to provide foundational data for reducing turnover intention by analyzing the relationship between violence experience and turnover intention, with a focus on the moderating effect of social support.
Methods: The participants were 284 EMTs affiliated with the Fire Safety Headquarters of G Metropolitan City, South Korea, who were career fire officials and actively engaged in emergency medical services at fire departments. Data were collected using a structured self-administered questionnaire from March 4 to March 18, 2025. Of the 284 questionnaires distributed, 275 were included in the final analysis after excluding 9 questionnaires due to insincere responses. The collected data were analyzed using IBM SPSS version 30.0. employing frequency and percentage, mean and standard deviation, independent t-tests, one-way ANOVA with Scheffé post hoc tests, correlation analysis, and hierarchical regression analysis were performed, and the level of statistical significance was set at p<.05.
Results: Among respondents, 36.7% experienced internal verbal violence, 14.9% experienced internal physical threats, and 2.2% experienced internal physical violence at least once during the past year. In addition, 100% experienced external verbal violence, 96.0% experienced external physical threats, and 66.5% experienced external physical violence. The mean score for social support was 3.93, and the mean score for turnover intention was 3.36. Correlation analysis showed that turnover intention was positively correlated with internal verbal violence and external violence factors, and negatively correlated with social support. Hierarchical regression analysis indicated that external verbal violence and external physical threat had significant positive relationships with turnover intention, whereas social support had a significant negative relationship within this association. In addition, the interaction terms of external verbal violence × social support and external physical threat × social support also had significant negative relationships with turnover intention.
Conclusion: In this study, external violence was a problem frequently experienced in the work environment of EMTs, suggesting that violence experience is not an individual or one-time incident but a problem that is continuously repeated. In addition, external verbal violence and external physical threats increased turnover intention among EMTs and functioned as major factors that threaten job continuity. In contrast, social support functioned as a moderating factor that alleviated the negative effects of these violence experiences and effectively reduced turnover intention. Therefore, in order to reduce turnover intention among EMTs, a parallel approach is required that includes prevention of external violence factors along with strengthening organizational social support systems.
