This study aimed to evaluate the economic feasibility of establishing a Health Promotion Center in Andong, a representative medically underserved area in South Korea. A cost-benefit analysis (CBA) from a societal perspective was conducted. The results showed that the present value of total benefits (₩23,182 million; approximately USD 16.6 million), significantly exceeded the present value of total costs (₩7555 million; approximately USD 5.4 million), yielding a net present value (NPV) of ₩15,627 million (USD 11.2 million) and an internal rate of return (IRR) of 27.9%. These findings provide empirical evidence that the establishment of the Health Promotion Center in Andong constitutes a highly economically viable public investment. To ensure its sustainable operation, strategies such as community-based program development, enhanced integration of local resources, and digital outreach efforts are essential. This study contributes empirical support for the expansion of Health Promotion Centers and offers a foundational reference for future policy formulation and resource allocation.
