치매와 실행증 평가 = Dementia and Apraxia test

다국어 초록 (Multilingual Abstract)

The purpose of this study is to investigate whether apraxia test can be used as screening test for dementia through a systematic review. The literature search focused on studies published for 10 years until December 2018 from Pubmed database. The search key words were “apraxia” and “dementia or Alzheimer”. Then based on the selection and exclusion criteria, 9 papers were selected at the end. In selected studies participants had a variety of underlying causes of dementia, and all participants with dementia represented significantly lower scores than healthy controls in apraxia test. The test used in each study was not the same but consistently used the method of imitation and pantomime and also included meaningful and meaningless movement items, And we could identify that apraxia test with buccofacial items can differentiate alzheimer disease from behavioral variant frontotemporal dementia. This suggests that apraxia test may be used as a screening test for dementia. In the future, it will be necessary to develop dementia apraxia test for screening.

국문 초록 (Abstract)

본 연구의 목적은 문헌 고찰을 통해 실행증 검사가 치매의 선별검사로서 이용될 수 있는지를 알아보기 위한 것이다. 2018년 12월까지 10년간 발표된 논문을 Pubmed에서 검색하였으며 “apraxia”, “dementia or Alzheimer”를 키워드로 조사하였다. 선정 및 제외조건에 따라 최종적으로 9편의 논문을 선정하였다. 선정된 연구들은 치매의 다양한 원인 질환들을 대상자로 하였고 모든 질환군들은 정상군보다 실행증 검사에서 유의하게 낮은 수행을 보였다. 각 연구에서 사용된 실행증 검사는 동일한 검사는 아니었으나 일관적으로 모방과 무언극을 통한 검사방법을 사용하고 의미가 있는 동작, 의미가 없는 동작을 포함하고 있었다. 그리고 안면 실행증 검사항목을 포함한 실행증 검사는 치매의 원인질환 중 행동변이형 전두측두치매와 알츠하이머형 치매를 구별할 수 있다는 것을 확인하였다. 이로써 실행증 평가는 치매의 선별검사로서 사용될 수 있는 가능성이 있다고 생각되며 국내에서 사용할 수 있는 치매환자를 위한 실행증 평가 개발이 필요할 것으로 생각된다.
