경피내시경하위루술 노인을 돌보는 부양자의 부양관련 삶의 질과 부양스트레스의 관계 = Correlation between Quality of Life and Stress of Caregivers who Support Elderly with Percutaneous Endoscopic Gastrostomy (PEG)

This study aimed to identify to the correlation between quality of life (QOL) and caregiving stress of caregivers who supported elders with percutaneous endoscopic gastrostomy (PEG). The data was collected using a structured-questionnaire and analyzed by PAWS 19.0 program using descriptive statistics, t-test, ANOVA, Scheffe post hoc, and Pearson's correlation coefficient. As a result, the mean score of QOL was 84 points and stress was 44 points. There were statistically significant differences in QOL and stress according to demographics (education, economic status, and burden of medical cost) and PEG related characteristics (reason to choose PEG). There was significant correlation between QOL and caregiving stress (r=-.645, p<.05). In conclusion, it is necessary to develop and apply characteristic-tailored nursing intervention in order to reduce stress and to improve QOL of elderly caregivers.

국문 초록 (Abstract)

본 연구는 경피내시경하위루술(percutaneous endoscopic gastrostomy, 이하 PEG) 노인 부양자의 부양관련 삶의 질과 부양스트레스의 관계를 확인하기 위한 서술적 상관관계 연구이다. 요양시설 및 재가 거주 PEG 노인 부양자 94명을 대상으로, Min, Lee, Kim, Suh, Kim(2000)과 Choi(1990)의 도구를 수정, 보완한 부양관련 삶의 질 도구와 Lee, Seo(1993)의 도구를 수정, 보완한 부양스트레스도구를 사용하여 자료를 수집하였다. 수집된 자료는 PAWS 19.0 program을 사용하여 기술통계, t-test, ANOVA, Scheffe 사후검정, Pearson 상관계수로 분석하였다. 연구결과, 부양관련 삶의 질은 평균 84점, 부양스트레스는 평균 44점으로 중간 정도였으며, 학력, 경제상태, 의료비의 부담 정도 등의 일반적 특성, PEG를 하게 된 이유 등 PEG 관련 특성에 따라 삶의 질과 부양스트레스에 유의한 차이가 나타났다. 또한 연구 대상의 삶의 질과 부양스트레스는 유의한 음의 상관관계(r=-.645, p<.001)에 있었다. 수명연장 및 의료기술의 발달로 PEG 등을 적용한 장기간의 경장영양이 가능해지면서 부양자의 의료비 부담 및 삶의 질 저하, 부양스트레스 증가가 확인된 본 연구결과를 토대로 향후 환자 및 부양자의 특성을 고려한 부양스트레스 감소 및 삶의 질 향상을 위한 중재 개발 및 적용이 요구된다.
