본 연구는 한국 여성들이 실제로 섭식장애를 경험하는 사례가 다수 존재하는 것에 비해, 의료체계 안에서 질병으로 진단받고 치료되지 않거나 혹은 이 과정에서 중도 탈락하는 경우가 현저히 많다는 문제의식에서 시작한다. 이에 그 원인을 한국 여성 섭식장애 당사자가 질병을 겪으면서 체득한 경험이 의료실천(medical practice)의 물리성(physicality)의 일환으로 취급되지 않는 데서 찾고자 한다. 정신질환이자 만성질환인 한국 여성 섭식장애 의료실천의 물리성을 다루기 위해서는, 병원 안의 물질성(materiality)인 장소성, 인간 및 비인간 행위자들의 물질성을 다룰 수 있어야 하고, 진료실 밖의 일상생활 질병 관리의 물질성 또한 진료실 안의 물리성으로 전환시켜 다룰 수 있어야 한다. 한국 여성 섭식장애 의료실천에서의 물리성 다루기는 실패할 확률이 높은데, 이는 여성성이라는 요소가 작용하기 때문이고, 이는 여성적 트라우마, 수치심이라는 구체적인 요소로 드러난다. 이를 바탕으로 한국 여성 섭식장애 의료실천을 물리성과 여성성을 중심으로 살펴보면, 의료실천과 당사자 경험 사이에 수많은 긴장이 있음을 알 수 있다. 이는 섭식장애의 회복(reparation), 고통, 질병 정체성 등과 관련한 열린 문제의식으로 드러난다. 이 문제의식은 단번에 해답을 내리기보다는 당사자 경험의 여러 상황과 맥락을 고려하여 풍부한 논의가 이뤄져야 하는 부분이다. 이를 위해 질병을 행하는(enact) 물리적 과정에서 일어나는 실천에 맞춘 아네마리 몰(Annemarie Mol)의 다중신체(Body Multiple) 이론과 한국 여성 섭식장애 당사자의 질병 경험을 바탕으로 한 수기 4편을 분석을 통해 보여주고자 한다. 질병을 통과한 당사자의 경험이 진료실 안에서 물리적 의료실천에 연루되는 과정을 밝히는 것은 정신질환이자 만성질환인 한국 여성 섭식장애의 현실적인 이해에 도움이 될 것이다. 본 연구의 주장은 세 가지로 요약할 수 있다. 첫째, 다른 모든 질병과 마찬가지로 섭식장애 또한 물리성을 기반으로 다뤄져야 의료체계 내에서 실행될 수 있다는 것이다. 섭식장애는 과도하게 먹거나 먹지 않고, 때로는 토하는 과정과 이로 인한 신체 지수 변화와 같은 물질적인 요소가 강조되는 측면이 있는 동시에, 물질적인 요소를 포착하기가 상대적으로 어려운 정신질환이자 진료실 밖의 일상생활 질병 관리 또한 중요하게 다뤄져야 하는 만성질환이기도 하다. 이런 요소들의 물질성을 의료실천의 물리성으로 다룰 수 있어야 섭식장애 의료실천의 문제를 개선할 수 있을 것이다. 둘째, 여성성이라는 요소가 한국 여성 섭식장애의 의료실천에 깊게 개입하는 과정에서, 당사자의 경험이 의료실천에서 오독되거나 소외되기 쉽다는 것이다. 섭식장애 의료실천에 개입하는 여성성은 구체적으로 트라우마와 수치심으로 나타난다. 이는 가부장제의 여성 억압에서 기인한 것으로, 섭식장애 의료실천에서 중요한 위상을 가지는 면담 과정에서 당사자 경험이 오독되거나 소외되는 요소로 작용한다. 셋째, 섭식장애 의료실천과 당사자 경험 사이의 긴장을 적절하게 다루기 위해서는 오히려 당사자의 ‘관점’을 벗어나 섭식장애 의료실천을 다중적(multiple)으로 봐야 한다는 것이다. 당사자의 관점을 벗어나겠다는 것은 당사자의 경험을 소외시키고 질병의 생의학적(bioedical) 진실만을 보겠다는 뜻이 아니다. 오히려 당사자의 경험이 의료실천에서 소외되지 않도록 하기 위해 ‘관점’들의 경합에서 벗어나겠다는 것이다. 이를 통해 섭식장애 의료실천과 당사자 경험 사이에 존재하는 회복, 고통, 질병 정체성 등과 같은 긴장을 열린 태도로 논의할 수 있다.

This study begins with the recognition that despite numerous cases of Korean women experiencing eating disorders, many do not receive a diagnosis within the medical system or drop out of treatment. The reason lies in the fact that the experiences acquired by Korean women with eating disorders during their illness are not treated as part of the physicality of medical practice. To address the physicality of Korean women's eating disorder medical practices, which are both mental disorders and chronic conditions, it is necessary to consider the materiality within hospital settings—such as spatiality—and the materiality of human and non-human actors. It should be possible to transform the management of daily life disease outside the clinic into a form of materiality within the clinic. However, handling the physicality within Korean women's eating disorder medical practice often faces high chances of failure due to the influence of femininity, manifested in specific elements such as feminine traumas and shame. Examining the physicality and femininity within Korean women's eating disorder medical practice reveals significant tensions between medical practice and the experiences of those individuals. This awareness presents an ongoing issue that requires nuanced discussions considering various contexts and scenarios of the individuals' experiences rather than seeking immediate solutions. To address this, the study proposes focusing on the enactment of illness through physical processes rather than dividing knowledge of illness into separate bodily and mental realms. Drawing on Annemarie Mol's theory of Body Multiple, based on experiences of those who have gone through illness, the study aims to enhance understanding of the practical reality of eating disorders as mental and chronic illnesses within Korean women's medical practice through an analysis of four four essays based on the firsthand experiences of the individuals. This study's arguments can be summarized into three main points. Firstly, like all other illnesses, eating disorders should be approached based on physicality to be effectively managed within the healthcare system. Eating disorders involve excessive or insufficient eating, sometimes accompanied by purging processes and resulting changes in body indices, emphasizing material aspects. Simultaneously, it is a mental disorder where capturing material elements is relatively difficult and also chronic conditions where managing disease in daily life outside of clinical settings is equally crucial. Addressing these material aspects within physicality in medical practice can improve the issues in eating disorder medical practices. Secondly, the element of femininity deeply influences the medical practices of Korean women with eating disorders, potentially leading to misunderstandings or marginalization of their experiences within medical practice. Femininity impacting eating disorder medical practices specifically manifests through traumas and shame, stemming from patriarchal suppression of women. These factors act as elements where the experiences of those affected may be misinterpreted or marginalized, especially in counseling processes crucial in eating disorder medical practices. Thirdly, to appropriately handle the tensions between eating disorder medical practices and the experiences of those affected, it is necessary to move beyond the perspectives of the individuals involved and view eating disorder medical practices in a multifaceted manner. Moving beyond the perspectives of those affected does not imply excluding their experiences and focusing solely on biomedical truths of the disease. Instead, it means moving away from competition among perspectives to ensure that the experiences of those involved are not marginalized within medical practice. This approach allows for open discussions on tensions such as recovery, suffering, disease identity, and others existing between eating disorder medical practices and the experiences of those affected.
