소아건강점검표의 사용을 통한 수술전 예비검사와의 유용성 비교 = (The) Comparison of Usefulness between Preoperative Screening Test and Child Health Check List

Background: Recently, the value of routine preoperative screening test has been questioned especially in healthy children scheduled for elective minor surgery. We have thought that perhaps enough history taking and physical examination may replace these test. We made the child health check list and parents to answer it. Then we compared the usefulness between preoperative screening test and child health check list.
Methods: Group A includes 67 children who's parents had answered the child health check list, group B includes 63 children who underwent routine preoperative screening tests. Each group was devided to ambulatory surgery patients(group A1, B1) and admitted patients for surgery(group A2, B2). Comparison values between two groups included preoperative evaluation time, operation time, anesthsia time and recovery time. Also clinical side effects were evaluated.
Results: The operation time, anesthsia time and recovery time in each group had no statistical differences. However there were statistical difference of preoperative evaluation time. It was significantly decreased in group A1(4.0 ± 1.2), A2(5.4 ± 1.3) compared to control group B1(7.6 ± 1.7), B2(8.6 ± 1.8).
Conclusion: The use of child heath check list saved time for preoperative evaluation and effort of anesthesiologist. It also decreased the discomfort of the children by omitting the invasive procedure and reduced the medical costs.

국문 초록 (Abstract)

배경: 최근 일률적 수술전 예비검사는 특히 계획된 소수술을 받게 되는 건강한 환아에게 그 필요성이 의문시 되고 있다. 이번 연구에서는 충분한 문진과 이학적 검사만으로 수술전 예비검사를 대신할 수 있을 수 있다고 생각하여, 이러한 검사를 대신할 수 있는 소아건강점검표를 만들어 보호자가 작성하게 한 후, 수술전 예비검사와 소아건강점검표의 사용을 비교해 보았다.
대상 및 방법: 충북대학교병원에서 정규 수술이 예정된 130명의 환아를 대상으로 하였다. 두 군으로 나누어 소아건강점검표를 작성하여 수술전 선별검사를 시행하지 않는 67명의 환아(group A)와 일률적 수술전 예비검사를 하는 63명의 환아(group B)로 이루어졌다. 또한 각 group A와 group B는 외래환자(group A1, group B1)와 입원환자(group A2, group B2)로 총 4 group으로 분류하여 분석, 비교하였다.
결과: 수술 시간, 마취 시간은 그룹 간에 통계적으로 유의한 차이가 없었다.마취과 의사에 의해 이루어지는 수술전 환자평가시간에서는 group A1(4.0 ± 1.2), A2(5.4 ± 1.3)가 group B1(7.6 ± 1.7), B2(8.6 ± 1.8)에 비해 유의하게 낮았다. 회복 시간의 차이는 없었으며 수술후 합병증 또한 유의한 차이를 보이지 않았다.
결론 : 소아건강점검표의 사용은 수술전 환자 평가시 마취과 의사의 시간과 노력을 줄여 줄 수 있고 검사를 두려워하는 환아들에게 침습적 검사를 피할 수 있고, 의료비를 절감을 기대할 수 있다.
