경계선 성격장애에 대한 변증법적 행동치료와 기독교 상담

국문 초록 (Abstract)

신앙공동체에 성격장애, 특히 경계선 성격장애는 관심을 갖지만 문제가 발생할 가능성이 높다. 성격장애와 경계선 성격장애의 특징은 정신장애의 진단 및 통계 편람의 새 개정판에 더 분명하게 제시된다. 성격장애는 자기정체감과 대인기능 능력의 훼손이다. 특히 경계선 성격장애의 특징은 감정적 불안정성, 자해, 분리불안, 불안감, 낮은 자존감, 우울성, 적대감, 공격성, 충동성, 그리고 해리취약성이다. 변증법적 행동치료에서 경계선 성격장애의 핵심 문제는 정서 조절이상이다. 이 정서 조절이상의 원인으로 비타당화 환경을 강조한다. 상담 전략에 양극단에서 정답을 찾는 것이 아니라 정답의 부재를 인정하고 중도를 찾는 변증법적 전략과 감정과 생각과 행동의 타당성을 인정하고 내담자를 격려하는 타당화 전략을 포함한다. 변증법적 행동치료의 인지행동주의적 방법과 기술을 지적하고 그것의 과도한 사용을 비판한다. 경계선 성격장애에 대한 변증법적 행동치료는 우리 기독교 상담에 도전한다. 전문화된 사역, 전문적 지식과 기술의 사용, 타당화 문화와 관계의 형성, 그리고 고통을 참고 기다리는 영성의 회복이 필요하다.

다국어 초록 (Multilingual Abstract)

Those with a personality disorder, particularly the borderline personality disorder(BPD), are attracted to a Christian congregation because they feel inner emptiness. However, their relationships with a member cannot be maintained because of their unstability. In DSM-5, personality disorder is newly defined as impaired sense of self and chronic interpersonal dysfunction while the features of BPD include emotional lability, self-harm, separation insecurity, anxiousness, low self-esteem, depressivity, hostility, aggression, impulsivity, and dissociation proneness. Linehan's Dialectical Behavior Therapy's therapeutic effectiveness of BPD is confirmed empirically. Its conceptualization of BPD entails that the core problem is emotional dysregulation. One of its strategies is the dialectical strategy that a middle way between BPD's polarities is worked out with recognizing that neither absolute truth nor relative truth exists but the truth evolves over time. Another is the validation strategy that communication of one's own feelings, actions, and cognitions are listened to and validated. Its overuse of cognitive behavioral methods and skills are illuminated and criticized. Linehan's Dialectical Behavior Therapy of BPD challenges contemporary Christian Counseling in Korea. It poses the needs for the professional Christian Counselor out of the local ministry, specific psychobiological knowledges and skills, validation culture and skills, and distress tolerance. Particularly tolerance and acceptance of subjective distress may evoke spirituality in contemporary Christian counseling.
