Objective: This study aimed to investigate the association between obesity and diabetic retinopathy among patients with type 2 diabetes mellitus using data from the Korea National Health and Nutrition Examination Survey (KNHANES).
Methods: A total of 2,429 patients with type 2 diabetes mellitus who underwent non-mydriatic fundus photography in the 2018–2021 KNHANES were included. Obesity was defined as a body mass index (BMI) ≥25 kg/m². Diabetic retinopathy was defined as the presence of retinal hemorrhage, exudates, or neovascularization observed in fundus photography. The association between obesity and diabetic retinopathy was evaluated using logistic regression models accounting for complex survey design weights. Multivariable models were adjusted for age, sex, marital status, education level, household income, smoking status, high-risk alcohol consumption, physical activity, hypertension, hypercholesterolemia, and glycated hemoglobin (HbA1c).
Results: The prevalence of diabetic retinopathy among patients with type 2 diabetes mellitus was 19.8%. The prevalence was 17.9% in the obese group and 21.8% in the non-obese group (p=0.020). After multivariable adjustment, obesity was significantly associated with a lower risk of diabetic retinopathy (OR, 0.79; 95% CI, 0.64–0.99; p=0.042). In contrast, higher HbA1c levels were significantly associated with an increased risk of diabetic retinopathy (OR, 1.52; 95% CI, 1.40–1.64; p<0.001).

Conclusion: These findings suggest a potential paradoxical protective association between obesity and diabetic retinopathy in patients with type 2 diabetes mellitus. Anthropometric indicators such as BMI may be important considerations in strategies for diabetic retinopathy prevention and management. Further longitudinal studies are warranted to clarify causality.
