만성 정신분열병 환자에서 비정형적 항정신병 약물 단독요법과 Sodium Valproate 병용요법 시 혈당 및 콜레스테롤 수치의 변화 = Changes in Glucose and Cholesterol Level in Schizophrenic Patients Treated with AtypicalAntipsychotics or with Valproate Combination

Objective
The association of obesity, diabetes mellitus, drug compliance, and body image with use of antipsychotic medication has been documented in many studies. We aimed to assess the effects of antipsychotics and sodium valproate combination therapy on glucose, cholesterol level and weight in schizophrenic patients.
Method
Chronic schizophrenic patients were grouped according to drug combination : risperidone, olanzapine, haloperidol, risperidone+valproic acid(VP), olanzapine+VP, haloperidol+VP. Planned assessment of included glucose and cholesterol, which were collected at baseline and at the end of the 12 weeks later.
Results
There were significantly increased glucose and cholesterol level, in olanzapine+VP group, but not in risperidone group. Cholesterol levels were not increased at the end of 12 weeks for haloperidol and risperidone group. Glucose and cholesterol levels had no significant association with weight gain.
Conclusion
Changes in glucose and cholesterol level, weight, and body mass index associated with antipsychotics including sodium valproate combination were reported in our report. In the future, we need follow up study and various studies about other combination drug and metabolic symptoms.

국문 초록 (Abstract)

연구목적：항정신병 약물은 당뇨병의 발병률을 증가시키고, 비만과 과체중에 의한 각종 부작용 뿐만 아니라 환자의 신체상에 대한 자신감 상실, 약물 순응도의 변화, 지지적 태도의 변화 등 각종 부작용을 일으켜서 환자의 치료와 예후에 영향을 미친다. 정신 분열병 환자에서 정형 및 비정형 항정신병약물과 sodium valproate의 병합요법이 혈당과 혈중 콜레스테롤의 수치, 체중 증가에 미치는 영향을 알아보았다. 방 법：정신분열병 환자만을 대상으로 하여 할로페리돌군, 리스페리돈군, 올란자핀군, 할로페리돌+sodium valproate(VP)군, 리스페리돈+VP군, 올란자핀+VP군으로 나누었다. 약물 투여 전과 약물 투여 12주 후에 혈당 및 혈중 콜레스테롤 수치를 측정하였다. 결 과：올란자핀+VP군에서는 다른 약물 군에 비해서 현격한 혈당의 증가를 보였으나 리스페리돈군에서는 유의한 혈당 수치의 증가를 보이지 않았다. 올란자핀+VP군에서는 다른 약물 군에 비해 유의한 콜레스테롤 수치의 증가를 보였고 리스페리돈, 할로페리돌 단독 사용 군에서는 다른 군에 비해서 혈중 콜레스테롤 수치의 변화가 적었다. 리스페리돈 군에서 가장 적은 체중 증가 및 체질량 지수의 변화를 보였고 올란자핀+VP군에서 가장 많은 체중 증가 및 체질량 지수의 변화를 보였다. 혈당의 변화와 체중의 변화와는 유의한 관계가 없었고 혈중 콜레스테롤의 변화와 체중의 변화에도 유의한 관계가 없었다.
결 론：항정신병 약물은 혈당 및 혈중 콜레스테롤 수치 뿐만 아니라 체중 및 체질량 지수에 영향을 끼치며 특히 sodium valproate 병용 요법에 있어서는 그 영향이 늘어나는 바 이에 대한 추적 연구 및 관심이 필요하며 향후 다른 병합 요법 약물과의 연구 및 증상의 연관성에 대한 연구가 필요할 것으로 생각된다.
