To investigate the functional and morphological changes of the kidney according to the changes of the serum concentrations of aminoglycoside aminoglycoside antibiotics, varing does of gentamicin(36, 54, 72, 90, and 108 mh/kg/day in group Ⅰ,Ⅱ,Ⅲ,Ⅳ, andⅤ, respectively) were injected intramuscularly to rats(3 times per day) for 5 and 10 days. One hour(peak serum concentration) and 8 hours(trough value) after final injections, serum concentrations of gentamicin, and serum creatinine and BUN levels were detected by fluorescence polarization immunoassay(FPIA) method and by automated chemical analyzer, respectively. Histological(hematoxylin-eosin, periodic acid Schiff, trichrome, and toluidine blue stains) and enzyme histochemical(acid phosphatase, succinic dehydrohenase, and adenosine triphosphatase) studies to the kidneys of the rats were also performed.
The results were as follows.
1. The peak serum concentrations and though values of gentamicin were optimal(5-10㎍/ml) in group Ⅰand Ⅱ. But in group Ⅲ, peak serum concentration was elevated that optimal concentration, and in group Ⅳ and Ⅴ, peak serum concentrations and trough values were elevated that optimal values.
2. Serum creatinine and BUN levels were gradually increased proportional to the amount of treated gentamicin.
3. On histological findings, degeneration and necrosis of the tubular epithelial cells were observed in group Ⅲinspite of optimal trough gentamicin value. But these morphological changes were increased progressively in severity proportional to the amount of gentamicin.
4. On enzyme histochemical findings, acid phosphatase, succinic dehydrogenase, and adenosine triphosphatase activities were decreased progressively proportional to the amount of gentamicin.
According to these results, degree of functional and morphological abnormalities of the kidney are roughly proportional to the trough gentamicin vlaues in serum, but we can't exclude that the renal abnormalities might be occurred in the acceptable trough of gentamicin. So we can conclued that the monitoring of the renal functions as well as drug monitoring are mandatory during gentamicin therapy, especially in patients with renal diseases.
