환자의 자기 이해와 내러티브적 참

국문 초록 (Abstract)

오늘날 다양한 학문영역에서 내러티브를 활용한 연구 및 내러티브 윤리에 대한 논의들이 활발하게 이루어지고 있다. 내러티브의 활용은 특히 환자의 자율성 존중이라는 생명윤리의 중요한 원칙과 관련하여 중요한 의미를 가진다. 왜냐하면 환자의 어떤 결정이 정말로 자율적인 결정이며 환자의 진정한 욕구를 반영하는 결정인지 분별해내기 쉽지 않은 상황들이 빈번하게 발생하기 때문이다. 질병과 관련된 자신의 경험에 대한 환자의 내러티브를 분석함으로써 환자의 자기 이해에 대한 단서를 얻는 것이 가능하다면, 그것이 의료진이나 환자의 가족으로 하여금 의료상의 중요한 선택들과 관련하여 환자의 자율적 뜻을 보다 잘 고려할 수 있도록 도울 것이라고 기대하는 것은 타당할 것이다. 그러나 내러티브의 활용 가능성을 모색하기에 앞서 선행되어야 할 것은 내러티브가 신뢰할 만한 것인지에 대한 고찰이다. 내러티브가 본성상 선택적이고 해석적이라는 사실이 내러티브의 오류 가능성으로 쉽게 연결될 수 있다는 것을 고려할 때, 이에 대한 고찰 없이 내러티브의 가치나 활용도를 이야기하는 것은 무의미할 수 있기 때문이다. 국내에서는 아직 내러티브의 신뢰성에 대한 연구가 미비한 실정이지만, 미국, 영국, 네덜란드 등의 국가에서는 그러한 연구가 내러티브적 참개념을 중심으로 활발하게 이루어지고 있다. 이 글은 이와 관련된 논의들 및 연구들을 검토하면서 내러티브의 오류 가능성에도 불구하고 우리가 생명의료윤리의 맥락에서 환자의 내러티브에 주목해야 할 이유를 밝히고, 그러한 내러티브가 우리에게 환자에 대한 어떤 종류의 이해를 줄 수 있는지에 대해 논의한다.

다국어 초록 (Multilingual Abstract)

There have been an increasing number of researches on narratives in bioethics recently, including the ones that explore the possibility of bioethics education through narratives, the ones that discuss narrative as a medium of virtue in medicine, and the attempts to acquire better understanding of patients by means of narrative analysis. Narratives can be especially important in relation to the principle of respect for autonomy, because they can provide some clues to the patient’s self-understanding when it is difficult to discern which decisions (or acts) are truly autonomous, and accordingly, which is to be respected. Considering the truth and accuracy issue inherent in narratives that are in nature selective and interpretive, the defense for the reliability, value, and truth of narratives are much needed. This article responds to that need by providing a careful examination of the discourse regarding self-understanding and narrative truth.
