다수의 소용종의 형태로 발현된 원발성 직장B-미만성 대세포 림프종 1예 = A Case of Primary Rectal Diffuse Large B Cell Lymphoma Presented as Multiple Polypoid Lesions

국문 초록 (Abstract)

원발성 직장 림프종은 드문 질환이며, B-미만성 대세포 림프종(diffuse large B-cell lymphoma, DLBL)의 조직형으로 나타나는 경우는 매우 드물다. 저자들은 대장암 수술 후 변비 증상으로 시행한 대장내시경에서 원발성 직장 림프종을 경험하여서 국내외 문헌고찰과 함께 보고한다. 내원 6년 전 구불창자선암으로 저위전방절제술을 받은 60세 여자환자가 변비 증상으로 내원하여 대장내시경검사를 시행하였다. 대장내시경에서 항문연 2 cm 상방에 다수의 작은 용종이 있어 시행한 조직결과에서 B-미만성 대세포 림프종을 진단받았다. 전산화단층촬영에서 다른 부위에 전이 소견이 관찰되지 않아 IE 병기로 진단되었으며, 환자는 항암치료 후 추적 내시경검사에서 용종이 발견되지 않았다.
