Objectives: This study aimed to examine the association between self-reported snoring and oral health and oral structural factors among Korean adults aged 40 years and older using data from the 8th Korea National Health and Nutrition Examination Survey (KNHANES, 2019–2021), and to evaluate whether these associations remained after adjusting for sociodemographic characteristics, health behaviors, and chronic disease–related factors.
Methods: Among 22,559 participants in the 8th KNHANES, a total of 5,344 adults aged 40 years or older were included in the final analysis after excluding individuals under 40 years of age and those with missing data on oral health, sociodemographic, or health behavior variables. The dependent variable was self-reported snoring status (yes/no). Independent variables consisted of 11 oral health–related factors, including subjective oral health status, toothache experience in the past year, history of orthodontic treatment, tooth damage experience, chewing and speaking difficulties, number of present natural teeth, and experiences of periodontal, restorative, endodontic, and prosthetic treatments. All analyses accounted for the complex sampling design of KNHANES and included weighted frequency analysis, cross-tabulation analysis, and stepwise complex-sample logistic regression models (Models 1–4).
Results: The prevalence of self-reported snoring among the study population was 20.9%. Cross-tabulation analyses showed significant differences in snoring status according to sex, age, alcohol consumption, smoking status, obesity status, neck circumference, hypertension, dyslipidemia, history of orthodontic treatment, and number of present natural teeth (p<0.05). In the complex-sample logistic regression analysis, Model 1, which included only oral health factors, showed that participants without a history of orthodontic treatment (OR=2.80, 95% CI: 1.39–5.65) and those with 20 or more present natural teeth (OR=1.35, 95% CI: 1.07–1.69) had significantly higher odds of snoring. These associations remained significant in the fully adjusted Model 4, in which the odds of snoring were higher among individuals without orthodontic treatment experience (OR=2.06, 95% CI: 1.04–4.11) and those with 20 or more present natural teeth (OR=1.32, 95% CI: 1.03–1.70). In addition, male sex, age group 50–59 years, current alcohol consumption, obesity (BMI ≥25 kg/m²), above-threshold neck circumference, hypertension, and dyslipidemia were significantly associated with snoring.
Conclusions: Among Korean adults aged 40 years and older, history of orthodontic treatment and number of present natural teeth were identified as oral health–related factors significantly associated with snoring, even after adjusting for sociodemographic characteristics and systemic health factors. These findings suggest that snoring may be associated not only with upper airway structural factors but also with oral health and oral structural characteristics, highlighting the importance of considering oral health factors in the development of snoring prevention and management strategies for middle-aged and older adults.
