Respiratory muscle fatigue is an important factor of respiratory failure in COPD patients. We studied the six week respiratory muscle training with resistive inspiratory device(Threshold) in 8 COPD patients to evaluate the effects on respiratory muscle performance. Pulmonary function and PImax as an index of inspiratory muscle strength were measured during the training period. Exercise performance was also measured using a cycle ergometer. No changes were noted in FVC, FEV_(1.0) and MVV but significant increase was noted in PI max (20.7%). Maximal exercise capacity represented by VE max and maximal work rate increased significantly (8.7%, 17% respectively). Exercise endurance represented by submaximal exercise duration increased significantly (14%). We concluded that the resistive inspiratory muscle training could not improve the pulmonary function but could improve the inspiratory muscle strength and exercise performance.
