It is evident that an elevation of urinary albumin excretion rate without clinical proteinuria strongly predicts a later progression of diabetic renal disease. Diabetic patients with normal or elevated resting albumm excretion rate show marked albuminuric response to physical exercise compared with matched non- diabetic controls. Employing treadmill test with exercise protocol of modified Bruce (stage II), we studied three groups: 7 normal controls, 11 type II diabetic patients (Group I) with normal urinary albumin excretion (AER<15 μg/ min) and 4 type 11 diabetics (Group II) with elevated urinary albumin excretion (AERa15 μg/min). The results are as follows: 1) In normal controls urinary albumin excretion rate was not increased after exercise. 2) In Group I patients (AER: 5.5±4.2 μg/min), urinary albumin excretion was increased to 7.8±10.8 μg/min after exercise. 3) In Group II (AER: 40.6±11.4 μg/min), urinary albumin excretion was increased to 92.0±35.8 μg/min after exercise, which was significantly higher than those observed in Group I. 4) Blood pressure and heart rate was not significantly different between normal controls and type II diabetic patients before and during exercise stress test. 5) There was poor correlation between diabetes duration or HgbAlc and maximal change in albumin excretion rate after exercise.
