Purpose: Fear of cancer recurrence (FCR) is one of the most prevalent psychological concerns among cancer survivors. This study aimed to examine the associations between FCR and quality of life (QOL), mental health, and health behaviors among survivors of six major cancer types in South Korea.

Methods: This cross-sectional study included 326 cancer survivors (stomach, colorectal, liver, breast, cervical/uterine, and lung cancer) recruited from four institutions between November 2023 and March 2024. FCR was assessed using a single-item measure, and participants were categorized into low and high FCR groups. QOL was evaluated using the EQ-5D, EQ-VAS, and EORTC QLQ-C30. Mental health was assessed using the Patient Health Questionnaire-9 (PHQ-9) and the Generalized Anxiety Disorder-7 (GAD-7). Health behaviors included changes in smoking and alcohol consumption, physical activity, health screening participation, vaccination, use of complementary and alternative medicine, nutritional supplement use, and weight control efforts. Data were analyzed using independent t-tests, chi-square tests, and logistic regression analyses.

Results: Participants with high FCR reported significantly poorer QOL, as indicated by lower EQ-5D index scores, EQ-VAS scores, and EORTC QLQ-C30 summary scores (all p < 0.001). They also exhibited significantly higher levels of depressive and anxiety symptoms, reflected by higher PHQ-9 and GAD-7 scores (both p < 0.001). In contrast, no significant associations were observed between FCR and health behaviors in crude or adjusted logistic regression analyses.

Conclusions: Higher levels of FCR were associated with poorer QOL and worse mental health among cancer survivors, whereas its association with health behaviors was limited. These findings underscore the importance of routinely assessing FCR and strengthening psychosocial support within survivorship care to improve overall well-being.
