돌발성 난청에서 고압산소요법의 적응증 확대 근거창출을 위한 임상시험 = Clinical trial for the treatment of sudden sensoryneural hearing loss using hyperbaric oxygen therapy

국문 초록 (Abstract)

연구의 목적 및 내용
□ 돌발성 난청 환자에서 고압산소치료용 의료기기의 적응증 확대를 위하여 돌발성 난청 환자들을 대상으로 안전성과 유효성을 검증하기 위한 임상시험을 진행하고 이의 결과를 바탕으로 신의료기술 평가를 신청함.
□ 임상시험 프로토콜 확립
○ 기존 문헌고찰을 통하여 적절한 중재군과 대조군을 선정하고 임상진료 프로토콜을 확립함.
○ 적절한 CRO를 선정하여 임상시험와 관리와 전반적인 사항을 논의함.
○ 임상시험 수행 의료기관의 돌발성 난청 내원 환자수와 통계전문가의 자문을 통하여 본 연구개발에 적합한 피험자 수를 산출함.
□ IRB/식약처 승인 획득
○ 의료기기 IRB에 심의를 거쳐 기관윤리위원회 승인을 획득함.
○ 임상시험 프로토콜에 대한 식약처 승인을 획득함.
연구개발성과
□ 임상시험 실시
○ 피험자를 모집하여 본 연구개발을 위한 임상시험을 진행함.
○ 돌발성 난청 환자를 대상으로 고압산소치료를 시행하여 초기 청력과 치료 후 청력을 비교하여 유효성에 대한 평가를 진행함.
○ 본 연구를 통하여 저주파 영역에서 고압산소 치료를 시행한 결과 통계적으로 유의하게 난청의 회복이 증가하는 것을 확인하였음.
□ 연구개발 성과
○ 논문작성 및 신의료기술 평가 신청 : 상기 임상시험을 통하여 얻어진 안전성과 유효성 결과를 바탕으로 신의료기술 평가를 신청하였음. 또한 위결과를 바탕으로 SCI논문을 개제하였음.
연구개발성과의 활용계획(기대효과)
□ 다양한 고압산소치료장비의 개발에 활용
○ 현재 의료용 고압산소치료 장비는 캡슐형 이외에는 국내에서 개발이 거의 이루어지지 않고 있으며 이는 임상진료에 활용하는 제한점으로 작용하고 있음.
○ 다양한 형태와 크기뿐만 아니라 다양한 압력을 전달할 수 있는 의료용 고압산소치료 장비의 개발이 필요한 실정이며 본 연구개발을 통하여 축적된 경험을 바탕으로 향상된 고압산소치료 장비의 개발을 진행할 수 있음.
□ 해외시장 진출
○ 현재 외국의 경우 돌발성 난청에 대한 고압산소치료를 활발히 시행하고 있으며 최근 이에 대한 연구결과가 다수 보고되고 있음.
□ 국산 의료기기 업체지원 및 새로운 시장 확보
○ 본 과제에 참여하고자 하는 의료기기 업체는 자체기술로 본 고압산소 치료장비를 제작하고 있으며 향후 장비의 성능향상을 통한 연구개발의 능력을 보유하고 있는 국내 극소수의 고압산소 장비업체 중의 하나임.
○ 돌발성 난청은 이비인후과 영역에서 중요한 질환 중의 하나로 국내에서 생산된 고압산소 치료장비가 이러한 질병의 치료에 대한 의료기기로 인정받는다면 대부분의 2차 및 3차 의료기관의 이비인후과에서 본 장비를 구비할 것이며 일부 1차 및 이비인후과 전문병원에서도 본 장비를 구비할 것으로 예상되어 국내에서 새로운 시장을 확보할 수 있을 것으로 예상됨.
□ 신의료기술 제도 활성화
○ 본 연구개발은 국산 의료기기를 이용하여 새로운 적응증을 대상으로 하여 신의료기술을 신청하고자 하는 연구개발로서 부족한 국내의 임상근거를 보완하여 신의료기술로 인정된다면 임상진료에 활용할 수 있어 신의료기술 제도의 활성화에 기여할 수 있을 것으로 생각됨.
