외국의 외래처방 제네릭 의약품의 입찰제 운영 현황과 시사점

국문 초록 (Abstract)

외래처방 의약품 입찰제는 약제비 절감 방안의 일환으로 뉴질랜드를 필두로, 유럽 6개국에서 시행되고 있으며, 특히 뉴질랜드의 경우 입찰제 대상 제네릭 의약품은 급여의약품의 절반에 해당하여 광범위하게 입찰이 진행되고 있는 것으로 파악된다. 이 글은 뉴질랜드 외에도 외래처방 의약품 입찰제를 실시하고 있는 네덜란드, 독일, 벨기에, 말타에서의 입찰제 담당기관과 적용 의약품 사례, 입찰제 운영을 원활하게 하기 위한 인센티브 제도 및 제도 효과와 문제에 대한 고찰을 수행하였다. 캐나다, 호주와 영국에서는 국가 정책으로서 입찰제 도입 타당성과 예상효과에 대한 검토를한 경험도 있어 향후 외래처방 의약품에 대한 규제 방안으로 확대가능성이 있다. 외래처방 입찰제 운영 결과 약가 인하를 통해 약제비 절감에 상당한 효과가 있다고 보고되고 있어, 2012년부터 시행된 “동일성분·동일약가 정책” 외에 별도의 제네릭 약가규제 방안이 부재한 우리나라에서 제네릭 약가를 추가적으로 인하시킬 수 있는 방안으로 도입을 검토할 필요성이 있다고 판단된다. 우리나라에서 약제비 절감을 위한 목적 외에도 의약품 입찰제를 통해 의약품 유통의 투명성을 달성할 수 있다는 부가적 장점이 있을 것으로 예상되어 우리나라 유통의 문제점을 해결해 줄 수 있는 방안으로 적극 검토해 볼만한 가치가 있다고 판단된다. 다만, 제네릭 의약품 품질과 효능에 대한 국민 신뢰도를 향상시킬 수 있는 방안을 먼저 선행하여야 하며, 제약산업에 미치는 영향을 고려하여 신중한 접근이 필요하다고 본다.
