본 연구는 노화 및 Angiotensin II (Ang II)에 의해 유도된 간 병리 상태에서 유산소 운동이 대사 관련 지방간질환(Metabolic-Associated Steatotic Liver Disease, MASLD) 지표에 미치는 영향을 규명하고자 하였다. 상이하지만 상호 보완적인 병리학적 상황을 다루기 위해 연구는 두 개의 장으로 구성되었다. 제1장은 99주령의 생쥐를 이용하여 노화에 따른 간 대사 변화 양상을 조사하였고, 제2장은 Ang II에 의한 스트레스 조건 하에서 유산소 운동이 간 반응에 미치는 영향을 분석하였다.
노화된 생쥐에서는 간 내 지방 축적과 함께 CLK2 및 PGC-1α의 발현이 증가하였으며, 특히 PGC-1α의 발현은 젊은 대조군에 비해 노화 대조군에서 유의하게 높았다. 평생 자발적 운동(Lifelong Spontaneous Exercise, LSE)은 CLK2의 발현을 유의하게 억제하였으며, 이는 운동이 간의 대사 신호 경로를 선택적으로 조절할 수 있음을 시사한다. 조직학적으로는 지방간의 특징이 관찰되었고, LSE 군은 NAS 점수가 더 낮게 나타나 운동이 간 병리 변화의 진행을 완화할 수 있음을 보여주었다.
Ang II 모델에서는 간 내 CLK2 발현이 유의하게 증가하였으나, 유산소 운동에 의해 억제되었으며, 특히 고강도 운동에서 가장 뚜렷한 억제 효과가 나타났다. 염증 지표인 iNOS의 발현 또한 운동에 의해 감소하였으나, IL-6 및 TNF-α의 발현은 그룹 간 유의한 차이를 보이지 않았다.
이러한 결과는 CLK2가 간 대사 조절에 중요한 PI3K/Akt 경로와 MASLD 유사 스트레스 상태를 연결하는 핵심 인자임을 시사한다. 일반적으로 CLK2는 인슐린 신호에 의해 활성화되지만, 노화나 레닌-안지오텐신계(RAS) 이상과 같은 병리적 상태에서는 PI3K/Akt 경로의 기능이 저해되어, 보상적 반응으로 CLK2 발현이 역설적으로 증가할 가능성이 있다. 그러나 대사 스트레스 및 운동 중 CLK2의 조절 기전에 대해서는 여전히 명확히 밝혀지지 않았으며, 이에 대한 추가 연구가 필요하다.
종합적으로 본 연구는 유산소 운동이 노화 및 Ang II 유도 간 스트레스 조건 하에서 CLK2의 발현을 조절할 수 있음을 보여주며, MASLD의 병태생리 및 운동의 분자적 효과에 대한 이해를 확장하는 데 기여한다. 향후 연구를 통해 CLK2를 포함한 조절 네트워크의 기전, 임상적 의미 및 치료적 가능성에 대한 보다 명확한 규명이 요구된다.

This study aimed to examine how aerobic exercise influences markers of metabolic-associated steatotic liver disease (MASLD) under hepatic pathological conditions induced by aging and Angiotensin II (Ang II). To address distinct yet complementary pathological contexts, the study was structured into two chapters: Chapter 1 investigated age-related hepatic metabolic alterations using 99-week-old mice, while Chapter 2 evaluated the effects of aerobic exercise on hepatic responses under Ang II-induced stress conditions.
In aged mice, hepatic lipid accumulation and the expression of CLK2 and PGC-1α were elevated, with PGC-1α expression being significantly higher in the aged control group compared to the young control group. Lifelong spontaneous exercise (LSE) significantly suppressed CLK2 expression, suggesting a selective modulation of hepatic metabolic signaling pathways by exercise. Additionally, Histological features of fatty liver were observed, and the LSE group also showed lower NAS scores, suggesting that lifelong spontaneous exercise may mitigate the progression of hepatic pathological changes.
In the Ang II model, hepatic CLK2 expression was significantly increased but was attenuated by aerobic exercise across both intensities, with high-intensity exercise showing the greatest suppressive effect. iNOS expression, a marker of inflammation, was also reduced by exercise; however, no significant differences were observed in IL-6 or TNF-α levels between groups.
These findings highlight CLK2 as a key node linking the PI3K/Akt pathwaycrucial for hepatic metabolic regulation to MASLD-like stress conditions. While CLK2 is known to be activated by insulin signaling, pathological states such as aging and RAS dysregulation may disrupt PI3K/Akt activity, potentially leading to paradoxical increases in CLK2 expression as a compensatory response. Despite this, the regulatory mechanisms of CLK2 under metabolic stress and exercise remain unclear, underscoring the need for further investigation.
Overall, this study suggests that aerobic exercise may modulate hepatic CLK2 expression under aging and Ang II-related stress conditions, contributing to the understanding of MASLD pathophysiology and the molecular effects of exercise. Further research is warranted to clarify the regulatory networks involving CLK2, their clinical implications, and potential therapeutic relevance in MASLD management.
