육아종성 충수돌기염으로 초기 발현한 크론병 1예 = A Case of Crohn’s Disease with an Initial Presentation of Granulomatous Appendicitis

국문 초록 (Abstract)

염증성 소화관 질환인 크론병은 소화관의 전장에 걸쳐 어디에서나 발병할 수 있지만, 병변의 원발 부위가 충수돌기인 경우는 세계적으로 그 보고가 매우 드물다. 충수돌기에 국한된 초기 크론병은 임상 양상이 급성 충수돌기염과 유사하며, 병리조직학적으로 다른 원인에 의한 육아종성 충수돌기염과 감별하기가 매우 어렵다. 따라서, 육아종성 충수돌기염의 원인이 크론병인 것을 확인하기 위해서는 충수돌기 이외 다른 부위에서 동시성 크론병의 존재 유무를 확인하거나 추적 관찰에서 이환 유무를 관찰해야 가능할 수 있다. 저자들은 26세 남자 환자에서 급성 충수돌기염으로 판단하고 외과적 수술을 시행하였으나 병리조직학적 소견에서 육아종성 충수돌기염으로 판명되어, 추적 대장내시경 검사를 통해 크론병의 이환 소견을 관찰하여 육아종성 충수돌기염의 원인을 크론병으로 진단하였기에 문헌 고찰과 함께 보고한다.
