목 높이 조절판을 이용한 기관내삽관 성공률 비교 = Comparison of success rates of endotracheal intubation using a neck height adjustment plate

국문 초록 (Abstract)

본 연구는 기관내삽관 시 환자의 자세인 냄새 맡는 자세와 특별 제작된 기구(ACE board)를 사용하여 만든 삽관 자세를 비교ㆍ분석하여 전문기도 술기 수행을 향상하는 데 목적이 있다. 연구대상은 전문심장소생술 및 전문기도 관리학을 이수한 N 대학교 응급구조학과 학생 30명을 대상으로 무작위 교차 방법(Randomized crossover study)으로 실험을 진행하였다. 연구 결과, ACE board를 사용하여 만든 삽관 자세에서 냄새 맡는 자세보다 더 나은 성문 시야를 보였으며, 그에 따른 성공률과 신속성에도 차이를 보였다. 성공률의 경우 ACE board를 사용하여 만든 삽관 자세는 냄새 맡는 자세에 비해 높았으나 유의한 차이는 보이지 않았다. 반면 신속성의 경우 ACE board를 사용하여 만든 삽관 자세는 냄새 맡는 자세보다 기관내삽관 속도가 더 빨랐으며, 통계적으로 유의한 차이(p<.01)를 보였다. 연구대상자의 주관적인 용이성, 자신감 그리고 선호도에서도 냄새 맡는 자세보다 ACE board를 사용하여 만든 삽관 자세에서 통계적으로 유의한 차이를 보여 향상된 결과를 나타냈다. 따라서 기관내삽관 시 환자를 ACE board를 사용하여 만든 삽관 자세로 취하면 전문기도 술기 수행을 향상하는 데 도움이 될 것으로 사료된다.

다국어 초록 (Multilingual Abstract)

The purpose of this study is to compare the sniffing position and the position by using a special ramp to improve the performance of advanced airway management during endotracheal intubation. A special ramp(ACE board) was designed and built for this study. The study involved 30 students from the Department of Emergency medical Service, N University. The randomized crossover study was conducted with the students who were assigned into two groups; Sniffing position and the position by using the ACE board. As a result of the study, the position by using the ACE board showed better laryngeal view than the sniffing position, and there were improvements in success and speed of intubation. In terms of the participants’ subjective ease, confidence, and preference, the position by using the ACE board showed better results than the sniffing posture. Therefore, it is considered that the position by using the ACE board during endotracheal intubation will improve the performance of advanced airway management.
