Purpose:
This study aimed to identify predictors of excessive daytime sleepiness (EDS) in patients with obstructive sleep apnea (OSA). Beyond physiological sleep parameters, obesity-related indices, laryngopharyngeal reflux (LPR) symptoms, and sleep-related functional outcomes were comprehensively evaluated to provide evidence for early prediction and management strategies for EDS.
Methods:
This retrospective study included 128 adults diagnosed with OSA by polysomnography at the Department of Otolaryngology, Chonnam National University Hospital, between January 2023 and December 2024. Study variables included general characteristics, apnea–hypopnea index (AHI), daytime sleepiness assessed by the Epworth Sleepiness Scale (ESS), LPR symptoms measured using the Reflux Symptom Index (RSI), and sleep-related functional status assessed by the Functional Outcomes of Sleep Questionnaire (FOSQ). Data were analyzed using binary logistic regression analysis, with a significance level set at 0.05.
Results:
Excessive daytime sleepiness in patients with OSA was more closely associated with obesity, laryngopharyngeal reflux symptoms, and impairments in sleep-related daily functioning than with physiological sleep indices such as AHI alone. In particular, obesity and LPR symptoms were identified as major predictors of EDS, while social and intimacy-related functional domains of the FOSQ were significantly associated with EDS occurrence. These findings suggest the importance of early identification of high-risk patients and the need for multidimensional management strategies incorporating weight management, control of LPR symptoms, and support for sleep-related functional outcomes in patients with OSA.
Conclusion:
Excessive daytime sleepiness in patients with OSA is more strongly associated with obesity, LPR symptoms, and functional impairment than with physiological sleep indices alone. BMI and LPR were confirmed as primary predictors of EDS, while social and intimacy-related functional impairments mediated its impact through quality-of-life pathways. These results highlight the importance of early identification of high-risk groups and the implementation of multidimensional intervention strategies—including weight management, LPR symptom control, and support for functional quality of life—to effectively manage EDS in OSA patients.
