Knowledge and Attitudes of Family Physician and Oncologist toward Cancer Pain Management in Korea

국문 초록 (Abstract)

연구배경: 본 연구는 일차의료를 담당하는 가정의학 전문의와 암 전문의 간의 암성 통증 치료에 관한 지식과 태도 조사를 통해 암성 통증 관리 장애요인을 밝히고자 하였다.
방법: 대한암학회의 회원 중에서, 실제 암환자 진료를 담당하고 있는 800명과 가정의학회 전문의 2,200명에게 마약성 진통제 처방에 대한 지식(13문항) 및 태도(6문항), 개인적 경험, 교육의 적절성과 장애 요인 등을 평가하는 설문지를 발송하였다.
결과: 설문을 발송한 대한암학회 전문의 800명 중 147명(18.4%)이 응답하였으며 가정의학회 전문의 2,200명 중 388명(17.6%)이 응답하였다. 암성 통증 관리의 지식에 대한 영역에서 응답자들은 특히, 마약성 진통제에 의한 호흡억제의 가능성, 마약성 진통제의 부작용에 대한 내성, 천장효과, 중독의 위험성, 보조 진통제의 사용 등에 대한 지식이 상당히 부족하였다. 다수의 의사들이 통증관리의 중요성, 통증 정도의 평가 주체, 치료에서의 우선 순위에 대해서는 긍정적인 태도를 보였으나 중독에 대한 염려, 최대 용량의 초기 투여에 대해서는 부정적인 태도를 보였다. 두 집단을 비교하였을 때, 가정의학 전문의가 암 전문의에 비해 지식의 13개 질문 중 9개 질문, 태도의 6개 질문 중 4개 문항에서 부적절한 지식과 태도를 보였다. 인구학적 변수와 암성 통증 환자의 진료 경험 등의 변수들을 보정하였을 때에는 혼돈의 부작용의 내성에 대해서만 암 전문의가 유의하게 높은 지식을 보인 반면, 항우울제의 진통효과에 대해서는 오히려 가정의학과 전문의가 더 바람직한 지식을 보였다. 또한, 가정의학 전문의가 통증 정도 평가의 주체와 치료에서 암성 통증 관리의 우선 순위에 대해서는 더 바람직한 태도를 보였다. 응답자 대부분이 암성 통증 치료의 의과대학 교육(94.0%), 전공의 수련(82.9%), 연수강좌(80.4%)가 적절치 못하다고 하였다. 암성 통증 관리의 장애요인에 대해서는 통증 관리에 대한 의사들의 불충분한 지식(66.3%), 부적절한 통증 평가(50.5%), 마약성 진통제 처방의 주저(42.7%) 등 의사측 요인과 전문적인 방법에 대한 접근성 부족(54.4%), 진통제에 대한 지나친 규제(44.3%), 다양한 진통제의 부족(39.2%) 등의 구조적인 요인들이 지적되었다.
결론: 다수의 의사들이 암성 통증 관리에 대해 상당한 지식 부족과 마약처방에 부정적인 태도를 가지고 있음을 확인할 수 있었다. 암성 통증 관리를 향상시키기 위해서는 의사들에게 지식과 태도에 대한 포괄적인 교육프로그램이 지속적으로 제공되어야 한다.
