병원내 자살 예방을 위한 입원환자 자살고위험군 선별/중재시스템 개발 = Development of screening/intervention system for the prevention of suicide in a medical setting

Purpose&Contents
The purpose of this study was to develop scale for assessing suicide risk. And we also aimed to develop screening/intervention system for high risk group of suicide in medical inpatients.
Results
① In the course of developing suicide risk scale for medical inpatients, we held several meetings at Korea university guro hospital to hear opinions from field doctors and employees regarding the dangers of suicides in hospitals.
② We developed the suicide risk scale for medical inpatients, and standardized and published in the journal Journal of Korean Medical Science, the SCI registration journal.
③ Using the developed scale, we conducted a pilot project to assess the risk of suicides in patients who were admitted to the Korea university guro hospital, and performed suicide prevention campaign to the patients, who were confirmed as high risk group of suicide.
④ We compared the suicide risk scale for medical inpatients, with previously used scale.
⑤ The medical institution carries out ' Job training for the promotion of the mental health of inpatients ' to on-site medical personnel.

국문 초록 (Abstract)

연구의 목적 및 내용
입원환자용 자살 위험성을 평가하는 평가도구를 개발하고, 이를 활용하여 입원환자 자살고위험군 선별/중재 시스템을 개발하여 시범운영 한다.
연구개발성과
① 입원 환자용 자살 위험성 평가 척도를 개발하는 과정에서 고려대 구로병원 적정진료관리팀과 수차례 회의를 개최하여, 병원 내 자살 위험성에 대한 현장 의료인 및 근무자들의 의견을 청취하였음.
② 입원 환자용 자살 위험성 평가 척도를 개발하였으며, 이를 표준화하여 SCI 등재 학술지인 Journal of Korean Medical Science에서 출판하였음.
③ 개발한 평가척도를 사용하여, 고려대 구로병원에 입원한 환자를 대상으로 자살 위험성을 평가하는 시범사업을 실시하였으며, 평가에 의해 자살 위험성이 있는 것으로 확인된 환자들을 대상으로 자살예방교육을 진행하였음.
④ 위③번의 시범사업을 실시하는 과정에서, 신규 개발한 평가척도를 기존에 고려대 구로병원에서 자살 위험성을 평가하기 위해 사용하여온 척도인, ‘병원불안 우울척도’및 ‘디스트레스 온도계’와 비교하는 연구를 진행하였음.
⑤ 현장 의료인을 대상으로 ‘입원환자 정신건강 증진을 위한 직무교육’을 실시하였음.
연구개발성과의 활용계획(기대효과)
1. 환자 안전 향상
병원의 의료진과 직원들의 교육과 예방활동을 통하여 자살예방에 대한 관심을 고취시켜 실제 고위험환자들의 초기 중재에 적극적으로 나섬으로 자살을 예방하고 나아가 퇴원 후에도 이러한 활동이 이어져 전체적인 자살률 감소에 기여할 수 있을 것으로 예상함. 또한 현재 기초 통계도 생산되지 않고 있는 병원내 자살사고에 대한 의료기관의 경각심을 고취하여 직접적인 원내 자살사고 예방에 기여할 수 있음.
2. 환자 안전 관련 연구 활성화
본 연구개발을 통하여 병원 내 자살사고를 예방하고 임상 진료 현장의 보건의료서비스 품질 및 시스템 환경을 개선시키고 환자 안전 관련한 연구를 활성화하고 저변을 확대할 수 있을 것으로 예상함.
