웹 기반 PBL을 적용한 온라인 의학 문제기반면담(PBI) 기술 모형 개발 = Online Medical Problem-Based Interview(PBI) Skill Model Applying Online Problem-Based Learning(PBL)

This study proposes a set of systematic procedures for the medical interview applying online Problem-Based Interview(PBI). This medical PBI involves the immersion of the doctor in the situation where the doctor applies acquired skills and knowledge towards the satisfactory resolution of the patient interview. This model combines Web-Based Instruction(WBI) with Problem-Based Learning(PBL) for the effective medical interview. In this model, the interpersonal skills between the doctor and the patient, the patient's problem examination, and the education and the administration for the patient adaptation to the treatment are mainly considered. This model consists of eight phases: the problem establishment, the close examination of symptoms, the close examination of patients, the integrated close examination, the clarification of the problem, the treatment method establishment, the close examination of the mutual role, the problem evaluation. The strategies for inspiring the motivation are applied to each phase. Especially, the process for elaboration is applied to the close examination of symptoms, the close examination of patients, and the integrated close examination. This model helps doctors and patients enhance the ability to solve the problem in the self-directed ways, and to collect and utilize information for the web-based medical PBI.

국문 초록 (Abstract)

본 연구는 웹 기반 의학면담에 적용하는 온라인 문제기반면담(PBI: Problem-Based Interview)기술 모형을 개발하여 환자중심의 만족스런 해결책을 강구 하기위한 것이다. 웹 기반 의학 PBI 모형은 웹 기반 학습(WBI)과 문제기반 학습(PBL)을 의학면담 상황에 맞추어 단계별로 구성한 것이다. 의사는 환자 중심 면담을 통한 객관적 판단 기반을 마련하며, 협동면담을 통한 환자 스스로의 해결능력을 배양하게 하여, 환자가 문제기반 면담과정을 통해 자발적 동기유발이 되어 스스로 결정하도록 돕게 된다. 문제기반면담 전략은 환자가 문제중심으로 면담을 하는 과정을 의사와 환자의 관계형성, 환자의 문제 진단, 환자가 치료에 잘 순응하도록 교육하고 관리하는 측면에서 수행되었다. 문제기반면담의 단계는 면담문제설정, 증상규명, 환자규명, 통합규명, 문제명료화, 치료방법설정, 상호역할규명, 문제평가의 8단계로 나누고 증상규명, 환자규명 및 통합규명에 대한 문제 심화과정과 단계별 동기유발전략을 적용하여 PBI 모형을 개발하였다. 이 모형은 환자의 자기주도적 문제해결능력 함양 및 문제해결을 위한 정보의 수집 및 이용기술을 상승시키는 데에 도움이 될 수 있을 것이다.
