국민건강영양조사 제7기 3차년도(2018) 진단의학검사 질관리 = Quality Control of the Clinical Laboratory for the Korea National Health and Nutrition Examination Survey(KNHANES) (2018, 7th third year)

Reliability of the medical laboratory test in the Korea National Health and Nutritional Examination Survey (KNHANES) is very important for the evaluation of prevalence of chronic disease such as hyperlipidemia, chronic kidney disease or diabetes. Therefore, strict quality control of the examination is required. Since 2008, this project has made an effort to improve the reliability of the examination results by evaluating the Seegene Medical Foundation (previously Neodin), which performs diagnostic medical examinations for KNHANES, through laboratory inspections, evaluation of internal and external quality controls, inter-laboratory comparisons, and traceability and reproducibility tests. In the 2018 KNHANES, four tests for Hg, Pb, Cd, Ni (urine) were excluded and no tests were added to the test items. Results of the evaluation of laboratory inspection, internal and external quality control were generally satisfactory. In summary, quality control has been improving for KNHANES to date. Continuous management, through evaluations such as laboratory inspections and by maintaining accuracy and traceability of the examinations by using reference institutes or reference material measurements, will be necessary.

국문 초록 (Abstract)

국민건강영양조사 사업에서 고지혈증, 만성콩팥병, 당뇨병 등의 만성병 지표의 정확한 산출을 위해서는 지표 산출의 근거 자료가 되는 진단의학검사 결과의 신뢰성 확보가 매우 중요하다. 따라서 검사의 엄격한 질 관리가 요구되는데, 본 과제는 2008년부터 현재까지 매년 국민건강영양조사의 진단의학검사 수행기관인 씨젠의료재단에 대해 현장실사, 내·외부정도관리, 검사실간 비교, 측정소급성 평가, 검체이중검사결과 등을 지속적으로 점검하여 검사 결과의 신뢰도를 높이고 있다. 2018년도에는 전년도 항목 중 Hg, Pb, Cd, Ni (urine)의 4 항목이 삭제되고, 추가된 종목은 없었다. 본 연구에서는 2018년도까지의 질 관리 현황을 알아보고 개선방향을 제시하고자 하였다. 현장실사 및 내·외부정도관리 평가의 경우 대부분 양호한 결과를 보였다. 종합적으로 판단하면 국민건강영양조사에 있어서 어느 정도 질 관리 수준이 향상되어 잘 관리되고 있는 것으로 생각되다. 따라서 앞으로도 지속적인 질 관리가 필요하겠고, 지속적인 정확도 혹은 검사의 소급성 확보를 위해서는 참고기관과의 비교나 표준물질 측정을 통하여 지속적인 관리가 필요하다고 생각된다.
