재가 여성노인의 우울, 체질량 및 인지기능이 주관적인 삶의 질에 미치는 영향: IADL 및 ADL을 매개변인으로 하여 = The Effect of Depression, BMI and Cognitive Function on Subjective Quality of Life in the Community-Dwelling Women Older Adults: Focused on the Mediating Effect of IADL and ADL

다국어 초록 (Multilingual Abstract)

This study examines the relationship between Depression, BMI, cognitive function and quality of life among elderly women, focusing on mediating effect of IADL and ADL. Elderly women aged≥65 years, living in community resident homes were included in the study. Based on survey data including 3,055 cases, structural equation modeling was conducted to analyze data. The results are as follows. 1. Depression of elderly people was association with IADL, ADL and quality of life in all samples. And also, BMI was association with ADL and quality of life. However, IADL and ADL completely mediated the relationship between cognitive function and quality of life in all samples. 2. BMI was association with ADL and quality of life in low depression group. Also, IADL completely mediated the relationship between cognitive function and quality of life in low depression group. 3. Only, IADL completely mediated the relationship between cognitive function and quality of life in high depression group. In conclusion, management of cognitive function and IADL are very important comparing to others in mental health service for elderly with depressive symptom and will help the elderly to feel the quality of their life.

국문 초록 (Abstract)

본 연구는 일상생활수행능력을 매개변인으로 하여 여성노인의 우울과 체질량(BMI), 인지기능이 주관적으로 지각하는 삶의 질에 미치는 영향에 대해서 알아보고자 수행되었다. 이를 위해 65세 이상 재가 여성노인 3천 55명을 대상으로 통계 분석하였다. 가설을 검증하기 위하여 구조방정식 모델을 사용했으며, 연구결과는 다음과 같다. 재가여성노인의 우울은 직접효과와 도구적 일상생활수행능력(IADL)과 일상생활수행능력(ADL)을 매개변인으로 한 간접효과로 주관적 삶의 질에 유의미하게 부적인 영향을 미쳤다. 또한 여성노인의 BMI는 직접효과와 ADL을 매개변인으로 하여 간접효과로서 주관적 삶의 질에 부적인 영향을 미쳤고, 인지기능은 ADL과 ADL을 매개변인으로 하여 주관적인 삶의 질에 간접적으로 긍정적인 효과가 있었다. 그리고 우울이 낮은 여성노인의 BMI는 주관적인 삶의 질에 직접영향을 미쳤고, IADL과 ADL을 매개로 하여 주관적인 삶의 질에 간접적으로 유의하게 부적인 영향, 인지기능은 IADL을 매개변인으로 하여 주관적인 삶의 질에 영향을 미쳤으나, ADL에 따른 매개효과나 주관적 삶의 질에 직접 영향을 미치지 않았다. 반면에 우울이 높은 여성노인들의 인지기능이 IADL을 매개로 한 경우에 주관적인 삶의 질에 영향을 미쳤다. 따라서 인지기능과 IADL 관리는 무엇보다도 우울한 여성노인의 삶의 질을 향상시키는데 매우 중요하다.
