크기 증가와 조영 변화를 보인 안와 내 종괴가 고립성 섬유종으로 밝혀진 1예 = Solitary Fibrous Tumor of Orbit: Changes of Tumor Size and Tumor Contrast Enhancement

국문 초록 (Abstract)

목적: 안와 내 종괴의 경과 관찰 중 영상의학검사에서 크기 및 조영증강의 변화를 보여 조직검사 결과 고립성 섬유종으로 밝혀진1예를 보고하고자 한다.
증례요약: 64세 남성이 어지러움, 고개 움직일 때 불편감, 후각이상, 안구돌출을 주소로 내원하였다. 종괴로 인한 증상 의심 하에 안와컴퓨터단층촬영검사를 시행하였고 검사 결과상 22×12×25 mm 크기의 균일한 조영증강을 보이는 우측 안와 내 종괴를 확인하였다.
종괴의 크기 및 안구 운동의 변화를 보면서 경과 관찰하였다. 2년 후 우안의 돌출 및 복시 증상을 주소로 재내원하여 시행한 안와컴퓨터단층촬영검사에서 종괴의 크기 증가와 이전보다 더 강한 조영증강이 관찰되었다. 진단 및 증상의 완화를 위해 전신마취 하에종괴의 절제 및 조직검사를 실시하였으며 조직검사 결과 고립성 섬유종으로 확인되었다. 수술적 치료 후 우안 눈꺼풀 부종 및 복시증상은 호전된 상태이며, 6개월 경과 후 수술적 부작용과 재발은 관찰되지 않았다.
결론: 고립성 섬유종은 안와에서 드물게 발생하는 연조직 종양으로 크기 증가와 영상 검사에서 조영증강의 변화를 동반한 안와 내 종괴에서 감별진단에 포함되어야 한다.
