뇌졸증 노인 주부양자의 우울 및 생활만족도 영향요인 분석 = Analysis of Depression and Life Satisfaction Predictors in Primary Caregivers of the Elderly with Stroke

The major purpose of this study is to find out predictors of depression and life satisfaction among primary caregivers of the elderly with stroke. Total 261 primary caregivers, who reside in 3 metropolitan city, completed self-administrated questionnaire or participated face-to-face interview. Three groups of plausible predictors are considered: the general characteristics of caregivers, caregiving situations, and individual and social resources. The differential effects of the 3 groups were analyzed by employing hierarchical regression analysis. First, general characteristics of the primary caregivers were entered in Model I. Variables related with caregiving situations were considered in Model II. Finally, factors associated with individual and social resources were included in Model III. This analytic approach produces the following findings. In Model I, positive past relationship with the elderly reduced depression level of the caregivers. In Model II, the caregiver's depression level increased when they took care of male patient and experienced higher level of patient's problem behavior. However, the quality of past relationship with the patients maintained the same relationship. Higher level of household income, better health status, active applying approach coping strategies were significant predictors in reducing caregiver's depression level in Model III. The effects of patient's gender and problem behavior on depression replicated in Model III. However, the effect of the past relationship was cancelled out. In the first model of life satisfaction, the quality of past relationship with the elderly also showed positive effect on life satisfaction. In the second model, male patients and increasing ADL problems impaired the caregiver's life satisfaction, while the past relationship maintained the same relationship. Constructive uses of coping strategies and more social support contributed to increase life satisfaction in Model III. Significant effects of patient's gender, ADL problems, and the past relationship on life satisfaction also appeared in Model III. The general patterns found in the major findings are consistent with the previous studies dealt with caregivers. However, this study suggests an unique implication from the overall findings. The QOL of the caregivers can substantially decline when older male stroke patients show higher level of behavior problem and dependency of ADL. The findings of this study provide an important clue for policy and practical interventions.

국문 초록 (Abstract)

본 연구는 3개 광역시에서 뇌졸중노인 가족부양자를 대상으로 이루어졌다. 연구목적은 가족부양자의 우울과 생활만족도 예측요인을 파악하고 기존의 장기요양보호노인 가족부양자에 대한 연구결과와 비교 차원에서 살펴보는 것이었다. 연구대상은 65세 이상 뇌졸중 노인의 주부양자였다. 독립변수는 가족부양자의 특성, 부양상황 특성, 개인적 및 사회적 자원의 세 영역이고, 종속변수는 우울과 생활만족도였다. 자료분석은 위계적 회귀분석을 실시하였다. 연구결과는 다음과 같다. 먼저 주부양자의 우울 예측요인을 보면, 모델 Ⅰ에서는 과거 노인과의 관계가 나쁠수록, 모델 Ⅱ에서는 남성노인을 돌보거나 환자의 문제행동이 증가할 때 우울이 증가하였다. 모델 II에서 과거관계의 질은 여전히 우울과 부정적 관계였다. 모델 Ⅲ의 분석결과, 주부양자의 높은 가구소득, 양호한 건강상태, 적극적 대처의 사용 및 사회적 지지가 우울을 감소시켰다. 모델 Ⅱ에서 유의미했던 피부양자의 성과 문제행동은 여전히 주요변수인 반면, 노인과의 과거관계의 질은 영향력이 상쇄되었다. 생활만족도를 종속변수로 한 예측요인을 보면, 모델 Ⅰ에서는 노인과의 과거관계가 좋을수록 생활만족도는 증가하였다. 모델 Ⅱ에서는 과거 노인과의 관계는 여전히 유의미한 예측요인이고, 피부양 노인이 남성일수록, 노인의 일상생활수행장애가 많을수록 생활만족도는 감소하였다. 모델 Ⅲ에서는 모델 Ⅱ의 유의미한 변수들이 그대로 유지되면서, 적극적인 대처전략의 활용과 사회적 지지의 증가가 생활만족도에 긍정적 영향을 미쳤다. 본 연구결과는 장기요양보호노인 또는 치매노인 가족부양자 대상 선행연구에서 나타난 예측요인과 유사한 점이 많다. 그러나 남성 뇌졸중노인을 돌보는 경우, 피부양자의 문제행동이나 일상생활 수행의 의존성이 높을수록 가족부양자의 삶의 질이 떨어질 수 있다는 결과는 뇌졸중노인 가족부양자 대상 정책적, 실천적 개입에 필요한 기초자료로서의 의미가 크다.
