중환자실에서 억제대 사용에 대한 생명윤리학적 고찰 = A Bioethical Study on the Use of Physical Restraints in Intensive Care Units

국문 초록 (Abstract)

신체 억제대는 중환자실에서 환자의 안전을 지키고 계획된 치료과 정의 유지를 돕기 위해 사용되는 간호 행위이며 중환자실 입원 환자 의 약 30 ～4 0 % 가 억제대를 적용하고 있는 것 으로 보고되고 있다. 본 연구의 목적은 중환자실에서 사용하는 억제대에 관한 생명윤리학적 고찰을 통해 그 문제점들을 분석하고 가능한 대안을 제시함으로써 우리 중환자실이 한층 더 인간 중심의 치료적 환경을 만들어 가는데 이바지하고자 함이다. 연구의 진행은 문헌고찰을 통해 중환자실에서 사용하는 억제대의 사용현황과 문제점을 확인하고 연구자가 중환자실 간호사로써 근무하면서 경험한 사례들을 중심으로 억제 대와 관련된 문제들을 생명윤리적학적 접근을 통해 분석하였다. 본 연구에서 제시한 세 가지 사례를 통해 억제대가 환자의 안전을 지키기 위한 선한 의도로 사용되지만, 방법 상 환자의 자율성과 존엄성을 침해 할 수 있으며 중환자실 입원 기간 중 간호사-환자 관계에 부정적 영향을 미칠 수 있다는 것을 확인하였다. 억제대를 사용함에 있어 생명윤리적인 문제가 발생하게 되는 것은 억제대를 적용하는 대상이 인간이라는 점 때문이며, 환자의 자율성과 존엄성을 유지시키고 환자가 가진 또 하나의 권리인 ‘안전’을 동시에 지키기 위해서 무엇보다 의료인에게 인간 본질에 대한 이해와 인간의 기본 권리에 대한 교육이 필수적으로 이루어져야 한다. 이러한 교육 을 통해 의료인은 억제대를 사용함에 있어 환자들의 개별적인 의학적 상황과 그들의 기본 권리를 고려한 판단 을 할 수 있어야 하고, 억제대를 사용하는 것 으로 인해 환자들의 자율성과 존엄성이 침해 받지 않도록 의료기관의 관 리자들은 지속적인 주의를 기울임 으로써 의료인의 억제대에 대한 인식과 태도뿐만 아니라 행위의 변화까지 유도해야 한다. 중환자실에서 사용하는 억제대의 제거가 이 연구의 궁극적인 목표는 아니다. 그보다는 환자의 자율성과 존엄함을 덜 제한하는 환경 속에서 환자들에게 안전하고 인간다운 간호를 제공하기 위해 의료진은 노력해야 할 것 이라는 점이다.

Physical restraints are a n u rsin g practice which appear to be away to protect patients and to maintain planned therapies in Intensive care units (ICU s). It has been reported that approximately 30 % to 4 0 % of ICU inpatient and apply physical restraints. The purpose of this study is to analyze bioethical issues with the use of physical restraints in ICUs and to contri bute to making ICUs more aware of the need to create a human centered environment. This study exa ines the current use of physical restraints in ICUs and the bioethical and moral questions thrugh a
review of the literature. Using a bioethical approach, these questions are addressed with a focus on the cases
that author has experiencd while working as ICU nurse. Three cases presented in this study prove that physical restraints can be used for a good purpose, bet that the way in which they are used can violate the patient's autonomy and dignity and negatively influence the relationship between patient and nurse. the bioethical problem occurs because restraints are used on human beings. Most of all,it is required for medical professionnals to understand the nature of human beings and to be educated about basic human rights in order to maintain the autonomy and dignity of the patient as well as protecting the patient 's safety. This education ill help medical professionals to be able to
determine the medical needs of each patient while respecting individual autonomy. In addition, administrators of medical facilities should pay permanent attention to the infringement of the individual rights during use of physical restraints and should be led in changing the perceptions, attitudes, clinical practice of medical professionals.The main purpose of the study is not to cause the removal of restraints from ICUs, bu t rather to create anenvironment
that does not restrict the patient's autonomy and dignity, while still providing safe and humanistic nursing to the patients.
