노인주간보호시설의 이용여부가 치매노인 주부양자의 부양부담에 미치는 영향 = The Effect of Caregiver's Burden for the Dementia Elderly by the Use of Daycare Center

The main purpose of this research is to assess the impact in the primary care giver's burden by the use of daycare facilities for the senile elderly. Survey research was used to find out whether the use of adult daycare centers has had any effect on the workload of caregivers who deal with dementia elderly. A total of 364 people answered the questionnaire which included 168 dementia elders' primary caregivers from 38 adult daycare centers in and around the Seoul and Kyoung-gi area, and 196 caregivers that have been caring for the dementia elders at home for more than 6 months and have not resorted to any adult daycare or in-home services. The caregiving burden, a dependant variable, includes 5 sub-categories that was measured separately: total caregiving burden and physical burden, social burden, emotional burden, financial burden, and negative change in family relationships. To find out whether the use of daycare centers affect the primary caregiver's caregiving burden, both the dementia elders' and primary caregivers' general characteristics, and social support characteristics have held under control, while a hierarchical multiple regression analysis has been performed to prove the plausibility of the use of adult daycare centers changing the workload on the primary caregiver. The result showed a decrease in their burden in all but one category, which was emotional burden, as a result of using adult daycare centers. This study presented specific policies and practical ways to reduce the workload on the primary caregivers as they cared for the elderly population suffering from dementia.

국문 초록 (Abstract)

노인주간보호시설의 이용여부가 주부양자의 부양부담에 미치는 영향력을 파악하기위해 주간보호시설을 이용하고 있는 치매노인 주부양자 168명과 재가 치매노인 주부양자 196명을 대상으로 설문조사를 실시하였다. 종속변수인 부양부담을 총 부양부담 및 신체적 부담, 사회활동적 부담, 정서적 부담, 재정적 부담, 가족관계의 부정적 변화 등 다섯 가지 하위차원으로 구분하여 측정하였다. 주간보호시설 이용여부가 주부양자의 부양부담에 미치는 영향력을 파악하기 위해 부양부담에 영향을 미치는 요인으로 밝혀진 치매노인과 주부양자의 일반적 특성 및 또 하나의 중요한 요인으로 언급되고 있는 사회적지지 특성을 모두 통제하고 주간보호시설 이용여부가 주부양자의 부양부담에 갖는 설명력을 검증하기 위해 위계적 다중회귀분석을 실시하였다. 그 결과, 노인부양으로 인한 주부양자의 분노, 우울, 소외감, 소진 등의 정서적 부담과 관련하여서는 부양부담 감소효과가 관찰되지 않았으나 이를 제외한 신체적, 사회활동적, 가족관계의 부정적 변화, 재정적 부담 및 총 부양부담에서 주간보호시설 이용이 실질적인 부담 감소 효과가 있음이 확인되었다. 이러한 결과는 주부양자의 부양부담 경감을 위한 방안으로 현재 실시되고 있는 노인주간보호가 실질적인 방안이 되고 있음을 의미하는 것이며, 그동안 부양부담의 감소 방안으로 다수의 연구자들에 의해 주장되어 왔던 주간보호시설을 비롯한 재가복지시설의 양적 확대 주장을 뒷받침하는 실질적인 근거자료를 제시하였다는 점에서 본 연구의 의의가 크다.
