This study explored the effectiveness of an integrated approach combining individual counseling and art therapy for a couple experiencing anxiety and relationship conflict due to communication difficulties. The participants were a couple in their fifties with a school-aged child, who took part in 16 weekly sessions(9 counseling sessions and 7 art therapy). The results of pre- and post-assessments using the Beck Anxiety Inventory(BAI), State-Trait Anxiety Inventory-Ⅰ,Ⅱ(STAI-Ⅰ,Ⅱ), Couple Communication Scale(CCS), and House-Tree-Person Test(HTP) test showed significant improvements. The husband’s BAI score decreased from 33 to 12, and the wife’s from 31 to 11. On the STAI, the husband’s state and trait anxiety scores decreased from 60 and 63 to 26 and 28, while the wife’s decreased from 56 and 58 to 22 and 24. On the Couple Communication Scale, the husband’s score increased from 54 to 97 and the wife’s from 68 to 102. In addition, the HTP test revealed clearer emotional expression and positive changes in interaction. Qualitative analysis further indicated improvements in emotional empathy and relationship satisfaction through collaborative work and mutual feedback. Overall, the case suggests that an integrated approach combining individual counseling and art therapy can be effective in reducing anxiety and promoting relationship recovery in couples. It also highlights the potential applicability of integrated interventions in clinical settings.
